Traits

Shapechanger. The werebear can use its action to polymorph into a Large Bear-humanoid hybrid or into a Large Dire bear, or back into its true form, which is humanoid. Its statistics, other than its size and AC, are the same in each form. Any equipment it is wearing or carrying isn't transformed. It reverts to its true form if it dies.

Keen Smell. The werebear has advantage on Wisdom (Perception) checks that rely on smell.

Pack Leader. The alpha's allies have advantage on melee attack rolls against any hostile creature within 5 feet of the alpha and the alpha isn't incapacitated.

Alpha's Presence (Hybrid Form Only). Any lycanthrope that starts its turn within 30 feet of the alpha's Hybrid form must succeed on a DC 21 Wisdom saving throw or be forced to use their reaction to transform into their own Hybrid form.

Legendary Resistance (3/Day). If the alpha fails a saving throw, it can choose to succeed instead.

Alpha Werebear Weaknesses. The Alpha Werebear has the following flaws:
Lunacy. The alpha werebear has disadvantage on wisdom saving throws when under the influence of the full moon. In addition, the werebear cannot turn into human form and must transform into hybrid form if currently in human form.
Silver vulnerability.
Wolfsbane. If a piercing weapon covered in wolfsbane causes injury, the alpha werebear takes 44(8d10) of necrotic damage. If wolfsbane is ingested, double the damage.
Shocking State. If the Alpha Werebear takes lightning damage, they cannot shapechange for a minute. The werebear stays in their current form.

Actions

Multiattack. In bear form, the werebear makes three claw attacks. In humanoid form, it makes three greataxe attacks. In hybrid form, it can attack like a bear or a humanoid.

Bite (Bear or Hybrid Form Only). Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 15 (3d10 + 7) piercing damage. If the target is a humanoid, it must succeed on a DC 14 Constitution saving throw or be cursed with werebear lycanthropy.

Claw (Bear or Hybrid Form Only). Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 13 (2d12 + 7) slashing damage.

Greataxe (Humanoid or Hybrid Form Only). Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 10 (2d12 + 7) slashing damage.

Potent virus. The Alpha Werebear bite requires a greater restoration spell to cure. A remedy can be made from wolf's bane (medicine DC 15) but must be administered before the next full moon. If a full transformation happens, it requires a wish spell to cure. A creature already contracted with lycanthropy is poisoned on a failed constitution saving throw. At the end of the creatures turn, they may make the saving throw again.

Legendary Actions

The alpha can take 3 legendary actions, choosing from the options below. Only one legendary action can be used at a time and only at the end of another creature's turn. The alpha regains spent legendary actions at the start of its turn.

Attack. The alpha makes a claw attack.

Intimidating Roar. The alpha lets out a furious roar, any creature within 30 feet of the alpha that can hear it, must succeed on a DC 20 Wisdom saving throw or be frightened until the end of the Alpha's next turn.

Mauling Charge (Costs 2 Actions). The alpha moves 20 feet towards a target and then makes a claw attack, on a hit, the target takes an extra 9 (2d8) slashing damage. If the target is a creature, it must succeed on a DC 21 Strength saving throw or be knocked prone.

Description

Werebears are powerful lycanthropes with the ability to temper their monstrous natures and reject their violent impulses. It fights with the ferocity of a bear, though even in bestial forms, it avoids biting so as to not pass on its curse. Typically, a werebear passes on its lycanthropy only to chosen companions or apprentices, spending the time that follows helping the new lycanthrope accept the curse in order to control it.

The Alpha Werebear is incredibly powerful and normally the protector of its kind. They don't hesitate to bring others of their kind to heel if they are a threat.
