Apathetic. Any spell, ability, effect, etc. that may alter or manipulate the being's mentality, such as directing aggression, causing fear, or controlling the being's mind are all nullified.
Disgusting stench. Any creature within 20 feet of the creature with less than 14 CON will spend their action and bonus action retching and possibly vomiting on each turn spent within the area, and cannot take reactions. It can be smelled within a 40-foot sphere, but only has this effect within 20 ft.
Infectious skin. Any humanoid creature that makes a melee attack against the creature or is hit by a melee attack from the creature is infected with Fleshrot (or Bloodskin Fever among experienced medics) and will begin to transform into a bloodskin shambler over the course of 3D4 days unless cured by a greater healing potion, or the Lesser Restoration spell. Characters with proficiency in medicine can make a medicine check against a DC of 14 to try to figure out what the creature is. over the course of infection, the target will experience rashes, hair loss, blisters all over the body, increased body temperature, deteriorating vision, and in the latest stages, intense burning pain, seemingly random tears in the skin and leaking blood, along with strange cancerous-looking growths. This will eventually culminate in the target becoming a mindless, bloodsoaked, gaunt, bloodksin shambler. If treated immediately however, simple antibiotic medicines, a paladin's Lay on Hands, or the Cure Wounds spell can cure the disease. This must be applied within 18 seconds (3 rounds).
Grow. Roll a D6. Each possible number rolled gives one of the results listed: -1: an extra, smaller arm sprouts out of an open "wound" on its torso. the extra arm grants 1 extra melee attack. -2: a section of the face begins to open up, becoming a mouth. This allows the exhale action. -3: the legs and arms extend slightly, adding +5 to the creature's speed, and 5 ft to the creature's reach. this can only happen once. -4: the rib cage expands, and starts convulsing like a slow heartbeat, granting +2 to constitution to the creature. -5: the limbs of the creature grow in mass, adding +2 strength to the creature -6: spores spill from the "wounds" of the creature, forcing a DC 14 constitution saving throw on every creature within a 15-foot radius, dealing 3D6 poison damage and leaving them poisoned for up to 1 hour on a failed save.
Strike. Melee Weapon Attack: +3 to hit, reach 5 ft., 1 target. Hit: 1D6 bludgeoning damage.
Exhale. In a 10-foot cone in front of the creature, a toxic spore cloud is released. All creatures within must make a DC 14 constitution saving throw or take 4D4 poison damage and be poisoned for up to 1 hour on a fail.
Description
The bloodskin shambler is a disgusting thing, being a once-sentient living person of nearly any humanoid race, horrifically transformed into a blood-soaked, nearly skeletal, seemingly sexless being with no distinct facial features at first glance, and is always completely void of any sort of clothing. It wanders around aimlessly, having no specific goal of any sort. They are incredibly rare, but when they are found, they are far from any sort of civilization, but not so much that they're only in completely remote areas, as they were once civilians.
These creatures are often mistaken for undead when found mindlessly wandering desolate or uninhabited areas, though they aren't at all. The afflicted individual never actually dies due to the infection, though their condition is horribly deteriorated and their bodies are grossly mutated. the openings in their skin and the blood covering their bodies is often what makes adventurers think of them as undead.
