Evisceratory Birth. If the bloater takes more than 10 points of piercing or slashing damage from a single attack 1d4 bloatlings crawl from within the damaged flesh.
Bloatling Incubation. The bloater has a maximum of 1d12+5 bloatlings inside its body at anytime. A bloater can gestate 1 bloatling every 6 hours.
Birth Bloatling. The bloater's thick hide ruptures in multiple points dealing 10 damage. 2d4 bloatlings spill forth from the wounds. Bloatlings take their turn directly after the bloater.
Final Sting. Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit: 46 (6d12 + 7) piercing damage. On a hit, the target creature must succeed on a DC 17 Constitution saving throw or become cursed. While cursed in this way, the creature gains one level of exhaustion per day, and cannot reduce their level of exhaustion by any means short of a wish spell. If a creature dies as a result of exhaustion the creature instead is transformed into a bloater. If a creature dies for any other reason the corpse continues to fester for a total of 6 days before it transforms into a living bloater. Whether the target succeeds or fails its saving throw the bloater using this attack dies. A spell like remove curse or greater restoration or similar magic can dispel the curse.
Defiant Sting. When an attack or damaging effect reduces the bloater to 0 hit points, the bloater may move its twice its movement in a straight line towards its nearest target. It may then use its Final Sting attack as a part of this reaction.
