Siege Monster. The Avatar of the Drowned deals double damage to objects and structures

Maddening Form. Creatures within line of sight of the Avatar must make a DC 12 Wisdom saving throw at the beginning of each of their turns. On failure, they take 2d8 psychic damage, and cannot take reactions this round.

Actions

Thrash (Recharge 5-6). The Avatar plunges his fists into the surface of the water, and summons forth a wave 50 feet wide, 6 feet long, and 10 feet tall that travels outward from a space in front of it. The wave travels 20 feet away from the Avatar at the end of its turn and then disperses. Creatures caught in the wave must make a DC 24 Strength save or be caught within it, traveling with it until it disperses, and taking 8d10 bludgeoning damage. Creatures that succeed take half damage, and are knocked prone.

Hand from the Deep. Melee Attack: +11 to hit, reach 15 ft., 1 target. Hit: 23 (3d8 + 8) bludgeoning damage. If the creature is medium or smaller, it is grappled (escape DC 20) and lifted 20 ft. into the air.

Devour. 1 target that is grappled by Hand of the Deep. The Avatar lowers a grappled creature into its maw and attempts to devour it. While inside of the Avatar, the creature is unable to breath or move, and takes 6d6 cold damage at the end of each of its turns spent within the Avatar's form. If a creature inside the Avatar deals 30 damage in a single turn to the Avatar, it will regurgitate the consumed creature, which lands prone within 10 feet of the Avatar.

Plunge. 1 target that is grappled by Hand of the Deep. The Avatar forces a creature underwater and attempts to crush and drown it. The grappled creature takes 3d8 bludgeoning damage and must make a DC 17 Constitution saving throw. On success, it manages to hold its breath underwater. On failure, the creatures lungs fill with water, and it begins to drown more quickly. At the end of their next turn, if it remains grappled and underwater, the target falls unconscious and is released.

Legendary Actions

The Avatar of the Drowned has 3 legendary actions, which may be taken at the end of another creature's turn. These actions refresh each round.

Lightning Storm (Costs 3 Actions). The Avatar calls bolts of lightning to strike up to 3 creatures within its line of sight, who are exposed to the sky. Targets take 2d8 lightning damage.

Hand (Costs 2 Actions). The Avatar makes a Hand from the Deep action.

Fling (Costs 1 Action). The Avatar tosses a creature it is holding at a point within 60 feet. The creature takes 1d6 damage for every 10 feet it is thrown, and an additional 2d10 damage if it strikes a wall or barrier.

Abyssal Souls (Costs 3 Actions). The Avatar lowers its head; from its beard, 1d4 Bloated Others emerge at random points within five feet of the Avatar.

Description

A churning silhouette rises from the depths, and water slips away to reveal a bony, sallow back. The emaciated form raises its head from the depths, gaunt and covered by deep green beard of kelp; figures and limbs are tangled among the strands of rotting hair. Two bony arms emerge from the abyss, each terminating in a skeletal hand with knifelike nails. 

Most horrible, though, is its face. Bloated, sickly skin; a misshapen, bald scalp ringed with more rotten, matted hair; a hungry, voidlike maw lined with sharklike rows of sawteeth; and where its eyes should be, two dead, glinting black pearls.


The Avatar of the Drowned represents the Old Man of the Ocean's influence and will. Like an avatar of a god, it is not the essence of the Great Old One itself; it's more akin to a puppet, pressing into the material world where the barrier is thin enough to manifest itself.
