Traits

Ambusher. The doppelganger has advantage on attack rolls against any creature it has surprised.

Surprise Attack. If the parasite surprises a creature and hits it with an attack during the first round of combat, the parasite can use Take Host as a reaction.

Latch On. If the parasite hits a creature with a melee attack, it can use a bonus action to attach itself to the creature. When the creature moves, the parasite moves with it. The parasite can also choose to detach itself (no action required).
A creature can use its action to remove the parasite, making a DC 12 Constitution saving throw as it does so. On a failure, the parasite can use Drain Will as a reaction.

Needle Fangs. When the parasite attacks a creature while hidden, the target must succeed on a DC 15 Wisdom (Perception) check or fail to notice the attack.

Sense Life. The parasite can sense the presence and location of any living creature within 30 feet of it that has an Intelligence of 3 or higher, regardless of interposing barriers.

Actions

Multiattack. The parasite makes one bite attack and uses its Drain Will.

Bite. Melee Weapon Attack: +2 to hit, reach 5 ft., one target. Hit: 5 (2d4) piercing damage.

Drain Will. The parasite attempts to drain the will of a living creature it is attached to that has an Intelligence of 3 or higher. The target must succeed on a DC 12 Wisdom saving throw against this magic or take 22 (5d8) poison damage. Also on a failure, roll 3d6: Reduce the targets Wisdom score by the total. If the total equals or exceeds the target's Wisdom score, that score is reduced to 0. The target is incapacitated until it regains at least one hit point.

Take Host. The parasite initiates a Wisdom contest with an incapacitated living creature within 5 feet of it. If it wins the contest, the parasite enters the target through the ear, mouth, or nose, and takes control of the target’s body. While inside a creature, the parasite has total cover against attacks and other effects originating outside its host. The parasite retains its Intelligence, Wisdom, and Charisma scores, as well as its telepathy. If the creatures Strength, Dexterity, or Constitution scores are below 18, they become 18. It knows everything the creature knew, including spells and languages. If the host body drops to 0 hit points, the parasite dies. A protection from evil and good spellcast on the body drives the parasite out into a space within 5 feet of it. The body then dies, unless its Health is restored within 1 round.
