Blood Frenzy. The blood fiend has advantage on melee attack rolls against any creature that doesn't have all its hit points.
Regeneration. The blood fiend regains 10 hit points at the start of its turn if it has at least 1 hit point, If the blood fiend takes radiant damage this trait doesn’t function at the start of the blood fiend’s next turn.
Innate Spellcasting. The blood fiend's spellcasting ability is charisma (spell save DC 18, +9 to hit with spell attacks). The blood fiend can innately cast the following spells, requiring no material components:
At will: detect evil and good, detect magic, teleport (self only)
3/day each: darkness. wrathful smite, blight
1/day each: power word pain, hallow
Multiattack. The blood fiend can make 4 attacks, only one of which can be a bite attack.
Bloodthirsty Bite. Melee Weapon Attack: +10 to hit, reach 5 ft., one willing creature, or a creature that is grappled by the blood fiend, incapacitated, or restrained. Hit: 12 (1d6 + 9) piercing damage plus 10 (3d6) necrotic damage. The target’s hit point maximum is reduced by an amount equal to the necrotic damage taken, and the blood fiend regains hit points equal to that amount. The reduction lasts until the target finishes a long rest. The target dies if this effect reduces its hit point maximum to 0. A humanoid of evil alignment slain in this way revives as a blood fiend on its next turn.
Claws. Melee Weapon Attack: +10 to hit, reach 5 ft., one creature. Hit: 17 (2d8 + 9) slashing damage. Instead of dealing damage, the blood fiend can grapple the target (escape DC 18).
Description
Blood Fiends were abominations who fed on the blood of living creatures. Initially created in the Elemental Chaos by the ancient primordial, Haemnathuun, they were just as prevalent in other planes, wherever there were creatures to provide food for them. Blood fiends were medium to huge creatures with a purple-black or reddish scaly hide, a lupine muzzle with great fangs, red eyes, and four clawed hands. Each of these hands had five razor-sharp claws extending from them, as did the feet. Blood fiends were also said by surviving scholars who witnessed them or had it reported to them, that these demons were so voracious for blood that they would give even the most ancient of vampires pause and even disgust.
Blood fiends preyed on other fiends, as vampires preyed on humans. They also had the same ability to transform other creatures into their kind. In order to assist them in their hunts, blood fiends could use a terror gaze that immobilized their unfortunate victim, making the feast all the easier for the blood fiend. Blood fiends themselves were immune to fear.
Most wounds inflicted on them would heal in a matter of moments.
These undead elemental creatures caught the eye of the mighty demon prince of undeath, Orcus, who after the death of the creator of the blood fiends, Haemnathuun, laid claim to them and asserted his will over them, adding them to his own forces and with it also claimed Haemnathuun's former title as "Blood Lord". So long as they did not attack him and followed his commands, Orcus was willing to let their feeding of other fiends in his realm go unpunished, as it did keep them busy while also furthering his goal to one day eliminate all other life from the multiverse, regardless what kind of life it might be, so that he may rule the remains and enjoy the sweat silence of a multiverse under his will unquestioned.
