Legendary Resistance (5/Day). If the dragon fails a saving throw, it can choose to succeed instead.

The scourge's beacon (10 charges): Ykiros, through the powerful lantern given to him by vecna, has access to the spells Disintegration (2 charges), Cloudkill (1 charge), dominate monster (3 charges), counterspell (1 charges, 2 for a level 5) and power word stun (3 charges). As long as the beacon still has charges, Ykiros has advantage on charisma, intelligence and wisdom saves and is immune to spells of level 3 or lower unless he decides otherwise. Hitting the lantern itself can cause it to lose charges: it has an ac of 25, every attack hitting it makes it lose one charge. An attack causing 50 or more damage makes it lose three charges.

Polymorph Ykiros can use an action to turn into another creature of challenge rating equal or inferior to his. He does not gain special abilities such as spellcasting when he does so. Ykiros can also change his size at will, to a maximum of 390 feet wingspan and 420 feets head to tail. Ykiros cannot use this ability while it has 400 health point or less.

The largest dragon. Ykiros's sheer size require him to use an incredible amount of strength to just move, strength that he can lack of if too much injured: If Ykiros's has 500 hp or lower, he cannot fly while in full size. If Ykiros has 250 hp or lower, its movement drops to 0.

Actions

Multiattack. The dragon can use its Frightful Presence. It then makes three attacks: one with its bite and two with its claws.

Bite. Melee Weapon Attack: +17 to hit, reach 15 ft., one target. Hit: 21 (2d10 + 10) piercing damage plus 14 (4d6) fire damage.

Swallow. after Ykiros makes one bite attack against a Large or smaller creature, if the attack hits, the target takes the bite's damage and must succeed a dc 19 strength save. If it fails, the target is swallowed. While swallowed, the creature is blinded and restrained, it has total cover against attacks and other effects outside the tarrasque, and it takes 49 (14d6) acid damage at the start of each of Ykiros's turns.

If Ykiros takes 50 damage or more on a single turn from a creature inside it, the tarrasque must succeed on a DC 23 Constitution saving throw at the end of that turn or regurgitate all swallowed creatures, which fall prone in a space within 10 feet of the tarrasque. If Ykiros dies, a swallowed creature is no longer restrained by it and can escape from the corpse by using 30 feet of movement, exiting prone.

Claw. Melee Weapon Attack: +17 to hit, reach 10 ft., one target. Hit: 17 (2d6 + 10) slashing damage.

Tail. Melee Weapon Attack: +17 to hit, reach 20 ft., one target. Hit: 19 (2d8 + 10) bludgeoning damage.

Frightful Presence. Each creature of the dragon's choice that is within 120 feet of the dragon and aware of it must succeed on a DC 21 Wisdom saving throw or become frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If a creature's saving throw is successful or the effect ends for it, the creature is immune to the dragon's Frightful Presence for the next 24 hours.

Fire Breath (Recharge 5–6). The dragon exhales fire in a 90-foot cone. Each creature in that area must make a DC 22 Dexterity saving throw, taking 91 (26d8) fire damage on a failed save, or half as much damage on a successful one.

World's end. While Ykiros is flying in its full size, it can use its action to make fire rain from his wings to the ground, burning and decimating everything in its path. Anything below him must make a dc 24 dexterity save. It takes (175) 50d6 fire damage on a failed save, half as much on a successful one. Additionally, while in its full size, Ykiros's breath weapon damage increases to 40d6 in a 180 foot cone.

Body Slam. While Ykiros is flying in its full size, it can use its action to slam into another flying creature, inflicting it 8d8+14 (50) bludgeonning damage. The target creature has to succeed a dc 18 constitution saving throw or be stunned for a round. If another creature is riding the target, it must succeed a dc 15 constitution save or be knocked out of the target.

Boiling blood. While Ykiros is in its full size, if it it has lost 400 or more hp, it can use its action to create two blood knights, to a maximum of four knights at all time. The blood knights are made of Ykiros's cursed blood, and use the knight parangon stats. If Ykiros dies or shrinks back to a smaller size while the blood knights are summoned, the blood knights disappear. If a blood knight is 60 feets away of Ykiros, it disappears.

 

Bonus Actions

The scourge's beacon (10 charges): Ykiros can use its bonus action to cast a spell from the scourge's beacon, expending the appropriate number of charges.

Legendary Actions

Ykiros can take 5 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The dragon regains spent legendary actions at the start of its turn.

Detect. The dragon makes a Wisdom (Perception) check.

Tail Attack. The dragon makes a tail attack.

The scourge's beacon (costs 3 actions). Ykiros uses the scourge's beacon to cast a spell.

Shake off (Costs 2 Actions). Ykiros violently moves, making every creature climbing on him have to succeed a dc 16 strength saving throw or fall from him, taking 2d6 bludgeonning damage plus any additionnal fall damage and be knocked prone. This dc raises by 2 while Ykiros assumes its full size, and by another 2 if Ykiros is flying, to a maximum of 20.

pin down. Ykiros makes a claw attack against a creature at his feet or climbing on his leg. If the attack hits, the target must make a dc 20 athletics or acrobatics check, or be pinned down under Ykiros's feet. A pinned down creature is incapacitated and knocked prone, and takes 5d6 bludgeonning damage at the top of its turn. After the damage is rolled, the pinned creature can make a dc 20 athletics or acrobatics check to try and break free. 

Wing Attack (Costs 2 Actions). The dragon beats its wings. Each creature within 15 feet of the dragon must succeed on a DC 25 Dexterity saving throw or take 17 (2d6 + 10) bludgeoning damage and be knocked prone. The dragon can then fly up to half its flying speed.

Description

The odor of sulfur and pumice surrounds a red dragon, whose swept-back horns and spinal frill define its silhouette. Its beaked snout vents smoke at all times, and its eyes dance with flame when it is angry.

Lair and Lair Actions

A Red Dragon’s Lair

Red dragons lair in high mountains or hills, dwelling in caverns under snow-capped peaks, or within the deep halls of abandoned mines and dwarven strongholds. Caves with volcanic or geothermal activity are the most highly prized red dragon lairs, creating hazards that hinder intruders and letting searing heat and volcanic gases wash over a dragon as it sleeps.

With its hoard well protected deep within the lair, a red dragon spends as much of its time outside the mountain as in it. For a red dragon, the great heights of the world are the throne from which it can look out to survey all it controls—and the wider world it seeks to control.

Throughout the lair complex, servants erect monuments to the dragon’s power, telling the grim story of its life, the enemies it has slain, and the nations it has conquered.

Lair Actions

On initiative count 20 (losing initiative ties), the dragon takes a lair action to cause one of the following effects; the dragon can’t use the same effect two rounds in a row:

  • Magma erupts from a point on the ground the dragon can see within 120 feet of it, creating a 20-foot-high, 5-foot-radius geyser. Each creature in the geyser’s area must make a DC 15 Dexterity saving throw, taking 21 (6d6) fire damage on a failed save, or half as much damage on a successful one.
  • A tremor shakes the lair in a 60-foot radius around the dragon. Each creature other than the dragon on the ground in that area must succeed on a DC 15 Dexterity saving throw or be knocked prone.
  • Volcanic gases form a cloud in a 20-foot-radius sphere centered on a point the dragon can see within 120 feet of it. The sphere spreads around corners, and its area is lightly obscured. It lasts until initiative count 20 on the next round. Each creature that starts its turn in the cloud must succeed on a DC 13 Constitution saving throw or be poisoned until the end of its turn. While poisoned in this way, a creature is incapacitated.
  • Ykiros can either raise 2 undead minotaurs and 3 banshees, an undead beholder and two bone nagas or an alhoon and an undead T-rex, depending on the phase of the fight.
