Traits

Eternal Mist. The Beast of the Eternal Mist generates a mystical fog that covers it's territory. This magical fog gives disadvantage on all Perception checks using sight or sound unless a creature has truesight. In addition the mist limits visibility to a range of 60 ft. regardless of a creatures abilities or traits unless it has truesight, tremorsense (on a ship), or an incredibly strong sense of smell.

Siege Monster. The Beast of the Eternal Mist deals double damage to buildings and structures.

Actions

Multiattack. The Beast of the Eternal Mist makes four attacks, one with it's bite, two with it's tentacles, and one with it's tail. The bite and tail attacks cannot target the same creature. The Beast of the Eternal Mist can use it's inhale instead of Bite, or a throw attack instead of a tentacle attack. Alternatively the Beast can replace any two of it's attacks with one of it's Heated Vapor attacks.

Bite. Melee Weapon Attack: +13 to hit, reach 10 ft., 1 target. Hit: 25 (4d8 + 7) piercing damage and the target is grappled if it is large sized or smaller. The Beast of the Eternal Mist can only have one creature grappled like this at a time. The grappled creature can escape with a succesful DC 19 Athletics or Acrobatics check.

Tentacle. Melee Weapon Attack: +13 to hit, reach 20 ft., 1 target. Hit: 25 (4d8+7) bludgeoning damage and the target is grappled if it is Huge sized or smaller. The grappled creature can attempt to escape by using it's action to make a DC 19 athletics or acrobatics check

Tail. Melee Weapon Attack: +13 to hit, reach 15 ft., 1 target. Hit: 29 (4d10+7) bludgeoning damage. The target of this attack must succeed on a DC 19 Strength saving throw or be pushed 20 ft. away from the Beast of the Eternal Mist. Creatures w/out a swim speed that are in the water have disadvantage on this saving throw.

Heated Vapor: Stream (Recharge 5-6). The Beast of the Eternal Mist exhales a 60 ft. cone of superheated steam similar to the mist that constantly surrounds it. Any creature within this area must succeed on a DC 19 Dex saving throw or take 77 (14d10) fire damage. A creature that is grappled in the Beast of the Eternal Mist's mouth automatically fails this saving throw and is no longer grappled.

Heated Vapor: Expulsion (Recharge 1-2). The Beast of the Eternal Mist exudes a cloud of the smae superheated steam. Any creature within 30 ft. of the Beast of the Eternal Mist must succeed on a DC 19 Con save or take 78 (12d12) fire damage. Any creature currently grappled by the Beast of the Eternal Mist has disadvantage on this saving throw.

Inhale. The Beast of the Eternal Mist makes a bite attack against a creature it has grappled. On a hit the target is inhaled down the Beast's throat and is no longer grappled. While inhaled a creature is restrained, blinded, and has total cover against any effects originating outside of the Beast of the Eternal Mist. While inhaled the creature takes 28 (8d6) fire damage at the start of each of it's turns. If the Beast of the Eternal Mist takes 45 or more damage in a single round from creatures it has inhaled it must succeed on a DC 23 Con saving throw or regurgitate all inhaled creatures which fall prone in an empty space within 15 ft. of the Beast of the Eternal Mist.

Throw. The Beast of the Eternal Mist makes a tentacle attack against a creature it has grappled in a tentacle. On a success the Beast of the Eternal Mist can throw the creature at a target of it's choice within 80 ft. of the Beast. The target and the thrown creature both take 25 (4d8+7) bludgeoning damage.

Reactions

Self Preservation. Immediately after regurgitating any creatures the Beast of the Eternal Mist can use it's reaction to move up to half it's movement speed without provoking opportunity attacks.

Description

At nearly 40 ft. long and 20 ft. wide the Beast of the Eternal Mist is a truly monstrous creature of the depths. The Beast of the Eternal Mist is poorly and inaccurately described many times due to the fact that it inhabits the Eternal Mists of Primordia. Only other very powerful elementals or very learned creatures of the deep know that the Mists of Primordia are actually generated by the Beast of the Eternal Mist itself. This generation of fog comes about when the ancient elemental being heats the water around it to near boiling. This ability is used to assist in the Beast's hunting providing near impenetrable visible and audible cover from it's prey. The Beast also uses this ability in combat both offensively and defensively if it feels threatened enough to do so. Looking from a distance like a very large shark or whale the Beast of the Eternal Mist has a gaping maw lined with rows of serrated teeth for tearing it's prey apart. Where pectoral fins would normally be the Beast has instead two long and very powerful tentacles it uses to hold it's prey still while it feasts. The tail of the Beast is well muscled and can be swung surprisingly quickly given the beasts massive size. Few encounter the Beast of the Eternal Mist and even fewer survive to tell of it. Those that do are often discarded as suffering from sea madness.
