Traits

Immeasurable Force. Any and all Strength and Constitution saves the Void Colossus takes automatically succeed.

Insurmountable. Spells using spell slots of 5th level and below do not effect the Void Colossus. 

Colossal. The Colossus can only take one step per turn, and it takes up a 10 ft. by 10 ft. area.

All Seeing Eye. Can see anything within 200 ft. without hindrance. Being blinded locks off Seeker Swarm from use.

Actions

Colossal Crush. Melee Weapon Attack: +10 to hit, reach 15 ft., all targets in a ten foot radius. Hit: 50 (3d20 + 10) Bludgeoning damage. 

Void Shriek. Range Weapon Attack: Will always hit, reach 120 ft., all targets in range. Hit: 25 (5d6) Thunder damage. All who are affected are frightened, all who are affected must make a Constitution save or be deafened. Deafened enemies who hear the shriek again take 1d6 psychic damage. 

Seeker Swarm. Range Weapon Attack: Will always hit, reach 120ft, 1 target. Hit: 25 (2d12 + 10) Void Damage. Fires a swarm of 1d20 Void Seekers that deals 25 (2d12 + 10) damage per Seeker. 

Bonus Actions

The Void's Wrath. If the Void Colossus successfully damages an enemy with its Crush, it may attack again. This goes on until the Colossus misses or the target dies. 

Reactions

Terror of the Void. If the Void Colossus sees an ally take damage it can Shriek in response.

Lidless Rage. If blinded, may use Void Shriek as a free action. 

Legendary Actions

Colossal Form. The Void Colossus may make three attacks each turn. Either two swings of its arm and a Shriek or two Shrieks and one swing. The Gazer can replace any of these actions with a use of Seeker Swarm.

Wrathful Eye. Can spend all three actions to fire Void Seekers at all enemies in sight. 

Description

A Void Colossus that has grown a single, large eye in the center of its headless form. It now houses swarms of ghostly beings called Void Seekers, that it directs as living missiles via an invisible targeting laser in its eye.
