Traits

Abyssal Plague. If a creature ingests any part of the dragon that creature must succeed on a DC 20 Constitution saving throw or contract the disease Abyssal Plague

Legendary Resistance (3/Day). If the dragon fails a saving throw, it can choose to succeed instead.

Unusual Nature. The dragon doesn’t require air, food, drink, or sleep.

Actions

Multiattack. The dragon makes one Bite attack and two Claw attacks.

Bite. Melee Weapon Attack: +11 to hit, reach 10 ft., one target. Hit: 17 (2d10 + 5) piercing damage plus 9 (2d8) necrotic damage. The target then must make a DC 20 Constitution saving throw, on a failed save the creature contracts the disease Abyssal Plague.

Claw. Melee Weapon Attack: +11 to hit, reach 5 ft., one target. Hit: 14 (2d6 + 5) slashing damage and 4 (1d8) necrotic damage.

Plagued Breath (Recharge 5–6). The dragon exhales a rotting plague cloud in a 60-foot cone. Each creature in that area must make a DC 20 Constitution saving throw. A creature that fails the save takes 45 (10d8) necrotic damage, contracts the disease Abyssal Plague, and their hit point maximum is also reduced by that amount. Creatures that pass the save take half as much damage and suffer no additional effects. If a creatures hit points is reduced to 0 as a result of taking this damage, they die and become an Abyssal Plague Zombie.

Description

A dragon that has succumbed to the abyssal plague doesn't simply die or spawn a demon, but instead becomes a fusion of undeath and draconic wrath.

Abyssal Plague. This disease and its malevolence was divine in nature, it was usually fatal unless cured or until the victim was transformed into a plague demon. Should the infection take root, the victim began to grow dark-red crystalline sores that appeared to be laced with silver and contained flecks of gold. These sores spread across the body, causing the victim to become even more contagious, but they also became more resilient and willful. Infected creatures gained both unnatural strength and a desire to destroy all living creatures. Eventually, the victim would die and their corpse would explode, creating a fully formed plague demon. Those who survived the plague and were cured said they experienced a vision of an enormously powerful entity trapped in a void of darkness and desolation. They heard the whisper of this entity, who filled them with a desire to destroy every living creature.

The simplest way to prevent the abyssal plague was to stay away from the Voidharrow or plague demons. Barring that, using divine magic was the only known way to prevent being infected by the disease. The only known way to cure the abyssal plague was exposing the victim to divine magic, but that only worked if the spell was cast before the plague had taken root in the victim's body at its later stages. The abyssal plague can be cured by any divine magic that would cure diseases.

Stages of the Abyssal Plague. The Abyssal Plague has three stages.

  • First Stage (0-30 days). The victim begins to grow dark-red crystalline sores that appeared to be laced with silver and contained flecks of gold.
  • Second Stage (30-40 days). The disease cannot be cured at this stage. The sores spread across the body, causing the victim to become even more contagious. The victims Strength, Constitution and Wisdom scores increase by 10. The victim grows a desire to destroy all living creatures.
  • Third and Final Stage (41 days). The victim dies and their corpse explodes, creating a fully formed Abyssal Plague Demon

If a creature carrying the abyssal plague were to die before transforming into a plague demon, that creature instead would become an Abyssal Plague Zombie
