Spellcasting. The Avatar of Talona is a 14th level spellcaster, its spellcasting ability is Charisma (spell save DC 20, + 12 to hit with spell attacks). It has the following spells available:
Cantrips (at will): detect magic, locate animals or plants, ray of sickness 
3/day each: Ray of enfeeblement, Counterspell, Blight, Contagion

1/day each: Finger of Death, Cloudkill

Magic Resistance. The Avatar of Talona has advantage on saving throws against spells or other magical effects. 

Magic Weapon. The Avatar of Talona’s weapon attacks are magical. 

 

Actions

Multiattack. The Avatar of Talon makes two attacks, one with its scythe and one with its claws.

Scythe. Melee Weapon Attack: +13 to hit, reach 10 ft., one target. Hit: 19 (2d12 + 6) slashing damage plus 9 (2d8) poison damage. 

Claws. Melee Weapon Attack: +13 to hit, reach 5 ft., one target. Hit: 15 (2d8 + 6) piercing damage plus 13 (3d8) poison damage. 

Anthrax cloud. The avatar of Talona releases a cloud of Anthrax spores that burst out and fill a 20 foot radius sphere centered on itself that lingers for one minute (10 rounds). Any living creature in the cloud when it appears or that enters it later must make a DC 19 Constitution saving throw. On a successful save a creature cannot be affected by this again for 24 hours. On a failed save the creature is infected with Anthrax and must roll a d4 to determine if it is cutaneous (1 or 3) or inhalational (2 or 4). The effects are outlined in the table below. The illness lasts until the creature is cured of the disease (either through magical means or the administration of IV antibiotics) or dies. The creature must repeat the saving throw every 24 hours, ending the infection on a success. On a failure, the creature’s infection worsens and after three failed saves the creature dies and is reanimated as an infested zombie under the control of the plague mistress. 

Cutaneous Anthrax: You are stunned for one round, after which you suffer no ill effects. Over the next 24 hours you develop red sores with central eschar over your body. 

Inhalation Anthrax: You are incapacitated for one round while you cough violently. After this you are Poisoned. You take a -5 to your constitution for each day you are affected by the disease. If your constitution reaches 0 before 3 days you die. 

Summon Demon (recharge 5-6). The Avatar of Talona spends its turn summoning a Chasme from the depths of the Abyss. The Chasme is controlled by the avatar, and begins its turn on initiative count 20. 

Reactions

Rebuke of the Plague Mistress. When the avatar is hit by an attack, it can spend its reaction to shriek in the direction of its attacker, casting Poison spray through its spittle. 

Legendary Actions

The Avatar of Talon can take 3 Legendary actions, choosing from the options below. Only one legendary action option can be used at a time, and only at the end of another creatures turn. The Avatar of Talona regains spent legendary actions at the start of its turn. 

Attack (Costs 1 action). The Avatar of Talona makes one attack with its claws or scythe.

Plague of Locusts (Costs 3 Actions). The Avatar of Talon calls forth a swarm of locusts that burst forth from its hand in a 30 foot cone that continues until it meets a solid wall. Ever creature in the path of the swarm must make a Constitution saving throw or take 3d10+6 piercing damage.  

Plague of Hail and Fire (Costs 3 Actions). The Avatar calls for a thunderstorm of hail and fire. Ice Storm is cast as specified in the spell, but instead of cold damage it is bludgeoning damage.  

Plague of Darkness (Costs 3 Actions). The Avatar casts Darkness centered on a point of its choosing. 

Plague of Boils (Costs 2 Actions). The Avatar casts Inflict Wounds at 3rd level, causing painful boils to appear on the skin of the affected creature. 

Plague of Frogs (Costs 2 Actions). The Avatar casts Cloud of Daggers at 5th level on a point of its choosing. Instead of conjuring knives, the spell conjures frogs. They inflict bludgeoning damage instead of piercing damage. 

Description

The Avatar of Talona is created when a subservient creature with whom she has made a pact, calls upon her protection against a great force trying to disobey her will. The summoning creature takes on the appearance of the goddess of poison and plagues, and gains the stats of her avatar. Upon the death of the avatar, the creature being possessed is returned to its true form with 1 hit point.
