Traits

Keen Smell. The baazrag has advantage on Wisdom (Perception) checks that rely on smell.

Pack Tactics. The baazrag has advantage on attack rolls against a creature if at least one of the baazrag's allies is within 5 feet of the creature and the ally isn't incapacitated.

Cruel Killer. The baazrag can use a bonus action at the end of its turn to make an extra attack action against a creature within 5 feet of it as long as that creature is frightened, prone, incapacitated, blinded, grappled, paralyzed, restrained, stunned, unconscious, or suffering from one or more levels of exhaustion.

Actions

Multiattack. The baazrag makes three attacks: one with its bite and two with its claws.

Claws. Melee Weapon Attack: +6 to hit, reach 5 ft., 1 target. Hit: 10 (2d6 + 4) slashing damage.

Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., 1 target. Hit: 12 (2d8 + 4) piercing damage, plus 4 (2d4) poison damage. If the target is a creature, it must succeed on a DC 15 Constitution saving throw or become poisoned. While poisoned in this way, the creature cannot recover Hit Points from a Short Rest, and can only recover Hit Points during a Long Rest by rolling half its maximum Hit Dice and adding its Constitution modifier to each roll. The creature regains Hit Points equal to the total. Every 24 hours that elapse, the poisoned creature can repeat the saving throw, ending the effect on itself on a success.

Frightful Howl. The baazrag lets out a bloodcurdling howl. Each creature within 150 feet of the baazrag that can hear this howl must succeed on a DC 15 Wisdom saving throw or become frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If a creature's saving throw is successful or the effect ends for it, the creature is immune to the baazrag's Frightful Howl for the next 24 hours.

Description

Baazrag are vicious pack animals that live in barren wastelands, feeding off of small animals and whatever unlucky traveler crosses their path. Utterly fearless and needlessly cruel, baazrag are covered in a tough hide and bony plating, and their sharp claws and toxic bite provide a deadly challenge to any who enter their domain. The infamous venom of a baazrag can harm the immune system and disrupt natural healing processes, but it is not dangerous by itself.

A pack of baazrag is led by one or more wasteclaw baazrag. These baazrag have grown exceptionally large and strong, and their bone-chilling howls are the bane of wasteland travelers, signaling an imminent baazrag attack. The hide of a wasteclaw baazrag is so tough that many weapons are useless against the beast, and like a rodent's teeth, a wasteclaw baazrag's titular claws never stop growing.
