Tentacles. Melee Weapon Attack: +11 to hit, reach 10 ft., one target. Hit: 13 (2d6 + 7) bludgeoning damage plus 1d8 acid damage from Black Bile which constantly seeps from the tentacles. The victim takes 1d8 additional acid damage the next round before te bile from that attack becomes inert.
Tail. Melee Weapon Attack: +11 to hit, reach 5 ft., one target. Hit: 15 (2d8 + 7) bludgeoning damage. The Target is grappled (DC 18)
Black Bile Breath (Recharge 5–6). The Bilewretch spits Black Bile in a 60-‐foot line that is 5 feet wide. When the bile is exposed to air it ignites. Each creature in that line must make a DC 18 Dexterity saving throw, taking 27 (6d8) acid damage and 21 (6d6) fire damage on a failed save, or half as much damage on a successful one. The bile clings and acid and fire continue to burn unless the victim uses an action to wash or scrape the bile off.
The Bilewretch can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The Bilewretch regains spent legendary actions at the start of its turn.
Detect. The Bilewretch makes a Wisdom (Perception) check.
Tail Attack. The Bilewretch makes a tail attack.
Eat Bilestone. The Bilewretch transfoms a 5ft cube of Bilestone into Black Bile of the World
Description
Monstrous aberrations sculpted from unknowable samples harvested eons ago from Holashner itself, bilewretches exist to perpetuate the transformation of black bile of the world into bilestone and then back again. Like an intelligent fountain, these monsters live only to endlessly break down bilestone and excrete it as liquid madness, protecting that which they have reformed. Often ordered by their masters to guard sites honoring the Elder Evils, these aberrations limited intellects myopically focus on their instructions.devising and honing the most effective methods of protecting that which they were created to honor.
Lair and Lair Actions
The Bilewretch resides in an area where Holashners Black Bile of the World seeps to the surface and keeps the bile from hardening into Bilestone by its presence.
Black Bile of the World is reeking fluid, thicker than oil but thinner than tar. It is the putrid excretion of the Elder Evil Holashner. In its purest form Black Bile (sometimes referred to as liquid madness) is among the foulest substances in existence: Rancid, caustic and flammable. Any creature that comes in contact with the viscous stuff takes 1d8 of acid damage on contact and another 1d8 of acid damage the next 2 rounds as the ooze clings and continues to burn. It can be scraped of or washed off with a large quantity of water. If submerged in Black Bile non-magical items are damaged by the acid.
The hardened Black bile of the World is called Bilestone. Proximity (30ft) to Bilestone gives all non-aberations disadvantage on wisdom saving throws and wisdom based skill checks.
