Traits

Legendary Resistance (3/Day). If the dragon fails a saving throw, it can choose to succeed instead.

Magic Resistance. The dragon has advantage on saving throws against spells and other magical effects.

Actions

Multiattack. The dragon makes three attacks: one with its Tentacles, and two with its Claws.

Claws. Melee Weapon Attack: +12 to hit, reach 10 ft., one target. Hit: 14 (2d6 + 7) slashing damage plus 4 (1d8) psychic damage. If the target is Huge or smaller, it is grappled (escape DC 18).

Tentacles. Melee Weapon Attack: +12 to hit, reach 15 ft., one target. Hit: 18 (2d10 + 7) psychic damage. If the target is a creature, it is grappled (escape DC 18). A creature grappled in this manner must make a DC 18 Intelligence saving throw or be stunned until the start of its next turn.

Extract Brain. Melee Weapon Attack: +12 to hit, reach 5 ft., one target grappled or stunned by the dragon. Hit: 55 (10d10) slashing damage. If the damage dealt would reduce the target to 0 hit points, it dies and the dragon regains 50 hit points as it extracts and devours the targets brain.

Spellcasting (Psionics). The dragon casts one of the following spells without providing any spell components, using Intelligence as the spellcasting ability (save DC 18):

3/day each: charm monster, command, dominate person, levitate, mass suggestion

Mind Blast (Recharge 5-6). The dragon emits psionic energy in a 60-foot cone. Each creature in the affected area must make a DC 18 Intelligence saving throw taking 49 (8d10 + 5) psychic damage and is stunned for 10 minutes on a failed save, or an affected creature takes half as much damage and isn't stunned on a successful one. An affected creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Bonus Actions

Bend Will. The dragon forces a creature it can see within 60 feet of it to make a DC 18 Wisdom saving throw. On a failed saving throw, an affected creature is considered an eligible target for the dragon's Extract Brain attack and the dragon can make said attack with advantage. If the dragon is currently in combat with the target, the save is made with advantage.

Reactions

Psionic Shielding. As a reaction to taking damage from a magical weapon, effect, or a spell, the dragon can reduce the damage dealt by 14 (4d6). If this reduces the damage dealt to 0, the attacker takes 10 (3d6) psychic damage.

Legendary Actions

The dragon can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The dragon regains spent legendary actions at the start of its turn.

Strike. The dragon makes a Claw attack.

Detect. The dragon makes a Wisdom (Perception) check.

Psionic Might (Costs 2 Actions). The dragon uses Spellcasting.

Psychic Overload (Costs 3 Actions). The dragon recharges, or uses its Mind Blast action.

Description

The adult illithidragon is so mighty that its mere presence begins to warp reality around it. Illithidragons of this age typically begin to give into the draconic narcissism and greed that others of their kind usually adhere to, and begin to plot against the elder brain of the colony. If the conflict begins to fall out of the dragon's favor, it will flee into the underdark to start its life anew, enslaving the inhabitants and warping the area around it.

Lair and Lair Actions

An Illithidragon's Lair

An illithidragon's lair is always underground, and often within the deepest regions the Underdark. The illithidragon makes its lair in great stalactite-filled cavern complexes with twisting tunnels. Its main den where it keeps its treasure hoard usually has chambers at varying heights, with at least four or more means of egress. The illithidragon keeps enslaved humanoids such as derro, dire corbies, and quaggoths to patrol its lair's outermost passages and forms partnerships with more powerful creatures such as ropers to guard its lair's inner areas.
The challenge rating of a legendary illithidragon increases by 1 when it’s encountered in its lair.

Lair Actions

On initiative count 20 (losing initiative ties), the illithidragon takes a lair action to cause one of the following effects; the illithidragon can’t use the same effect two rounds in a row:
Telekinetic Outreach.  The illithidragon casts telekinesis (no components required) While maintaining concentration on this effect, the illithidragon can’t take other lair actions.
Overpowering Will.  One creature the illithidragon can see within 120 feet of it must succeed on a DC 18 Wisdom saving throw or be charmed by the illithidragon as if the dragon had cast dominate monster on it. The effect ends on initiative count 20 on the next round.

Regional Effects

The region containing a legendary illithidragon’s lair is warped by the illithidragon’s magic, which creates one or more of the following effects:
Psionic Whispers.
Non-evil creatures within 6 miles of the illithidragon’s lair randomly hear whispered voices in their heads.
Otherworldly Adaptation.
Plants, fungi, and animals within 1 mile of the of the illithidragon’s lair have twisted, aberrant forms and tentacles.
Psychic Nightmares.
Whenever a creature sleeps or enters a state of trance within 6 miles of the illithidragon’s lair, it experiences a haunting nightmare and wakes up, its rest interrupted.
If the illithidragon dies, the nightmares and whispers immediately end while the plants, fungi, and animals return to their normal forms over the course of 1d10 days.
