Immutable Form: The Animated Medical Waste is immune to any magical effect that would alter its form.

Spider Climb: The Animated Medical Waste can walk across walls and ceilings without needing to make an ability check.

Long Reach: The Animated Medical Waste’s melee range is 20 feet. Creatures provoke opportunity attacks from it when they exit its 20 foot range.

False Appearance: While completely still, the Animated Medical Waste is indistinguishable from a large pile of bloody bandages, filthy bedsheets and various discarded medical implements.

Made of Bandages: Being made mostly of bandages, any creature, object or effect that would grapple or restrain the Animated Medical Waste will instead tear off a small chunk of it, dealing 1d6 damage (ignoring all damage resistances, vulnerabilities and immunities) and leaving the Animated Medical Waste un-grappled and un-restrained. This does not apply if the Animated Medical Waste is completely contained, engulfed or swallowed.

Stench of Disease: Any creature that engulfs or swallows the Animated Medical Waste, or comes within 15 feet of the Animated Medical Waste, or starts its turn within 15 feet of the Animated Medical Waste, must make a DC 16 Constitution saving throw. On a failed save, the creature must use its movement to exit the 15-foot radius before it takes any other actions. If the creature’s movement is not enough to exit the radius on this turn, it must use its action to dash and continue moving away from the Animated Medical Waste. On a successful save, the creature becomes immune to this effect for the next 24 hours. Any creature which does not need to breathe, does not have a sense of smell, or is immune to poison and disease is immune to this effect.

Contaminated: Any creature that swallows or engulfs or is damaged by the Animated Medical Waste must make a DC 13 Constitution saving throw or become Poisoned. The creature may repeat the saving throw at the end of each long rest until the effect is cleared.

Actions

Multiattack: The Animated Medical Waste makes three attacks when it takes the attack action on its turn.

Surgical Hail: Ranged Weapon Attack, 40ft/120ft range, +7 to hit, one target. Hit: 10 (1d12 + 4) slashing damage + 10 (1d12 + 4) piercing damage.

Bandage Sweep: All non-flying creatures in a 10ft x 10ft area (within the Animated Medical Waste’s 20 foot melee range) must make a DC 15 Dexterity saving throw or receive (1d10 +1) bludgeoning damage and fall prone. Creatures in the area may also choose to make a Strength saving throw instead but are guaranteed to receive the damage component of the attack even if they succeed on their saving throw against falling prone.

Anaesthetic: Melee Weapon Attack, 20 ft range, +6 to hit, one target. Hit: 19 (2d8 +10) piercing damage and the target must make a DC 13 Constitution Saving throw or fall prone. If the creature is already prone, a failed save will instead cause them to be Paralysed until the beginning of the Animated Medical Waste’s next turn.

Bandage Choke: One target chosen by the Animated Medical Waste within 20 feet must make a DC 16 Strength or Dexterity saving throw or receive 12 (1d12 +6) bludgeoning damage and be pushed 15 feet towards or away from the Animated Medical Waste (its choice) and fall prone.

Description

It is a well-known fact that any institution that practices magic must be careful of how it disposes of its waste, as stray magic from discarded or misplaced magical items can cause all sorts of awkward and inconvenient effects. Medical institutions are no exception to this rule – in fact, any hospital worth visiting will be an avid follower of it. One of the biggest reasons why is the risk of the medical waste becoming animated. This malevolent swirling mass of bloody bandages, misplaced scalpels, dirty needles, infected blankets and other…fluids… is hostile to everything that it can perceive, killing and rampaging until it is destroyed. Not great for the hospital’s image.
