Its statistics, other than its AC, are the same in each form. Any equipment it is wearing or carrying isn't transformed. It reverts to its true form if it dies.

Amphibious. The werefrog can breath air and water.

Acid Zone. If the target is hit with acid spit. Everyone in a 5ft radius must make a DC 10 Dexterity save or take 1d6 acid damage.

If the target is a humanoid, it must succeed on a DC 10 Constitution saving throw or be cursed with werefrog lycanthropy.

Bonus Actions

If the target is hit with tongue grapple the werefrog can retract it’s tongue back to itself along with the target, holding the target in it’s mouth. If the target is in the werefrogs mouth they must make a DC 11 Strength saving throw or be restrained. For every 1 minute the target is in the werefrogs mouth they take 1d6 acid damage and remain restrained till the DC is met.

Reactions

Leap. (Hybrid Form or Giant Frog Form). If the werefrog takes 10 or more damage. The werefrog can make a leap up to 15ft (half its distance) away from the target. Upon doing so the werefrog does not provoke an attack of opportunity.

Description

Werefrogs tend to be more cunning, quick, type. Hanging back and roping up it’s prey with its tongue. In humanoid form they tend to sit around and try to blend in with everyone around them. Normally not fitting in when their eyes dart around as well as their tongue hanging out of their mouth. Werefrogs tend to keep one or two friends close to them. Mostly because where you will find them is deep swamp areas.
