Death Throes:
When reduced to 0 hit points or below, the Blacktooth Devourer's body is no longer held together by supernatural force and begins to dissolve into a dark, sticky, rank-smelling puddle, emitting small bursts of infected pus. After 2 rounds it will be fully liquefied. Any living creature making contact with the liquid must make a DC 12 CON save. On a successful save, the character vomits explosively and has disadvantage on their next attack. On a failed save, affected creature suffers the effects of a Slow spell for 1d6 rounds and experience narcotic effects.
Insatiable Hunger: If a Blacktooth Devourer is not fed for 2 consecutive days, it will go berzerk, its intelligence dropping to zero, and will attempt to escape to find food. If it cannot escape, on the third day, it will expire, and enter the Death Throes state as described above.
Multiattack. The Blacktooth Devourer has a claw attack and a follow-up bite attack IFF the claw is successful, using the purchase gained from the successful claw.
Melee attack - Claw. Unarmed attack: +6 to hit. 1d6+2 slashing damage.
Melee attack - Bite. +6 to hit. 2d6 damage and DC12 CON save. On a successful save: target vomits explosively, has disadvantage on next attack. On failed save: target is affected as Slow spell for 1d6 rounds as they suffer narcotic effects.
Description
A human/demon possession hybrid dreamt up by a truly twisted mind, when a human or half-elf becomes addicted to Tambrush, a powerfully addictive narcotic, they will eventually slip into a coma nearly indistinguishable from death, unless intervention occurs.
Coma victims are then recovered by cultists, who prepare a special Binding Ritual spell and summon a cooperative hezrou demon’s spirit to reside within the host body. The host’s personality is trapped inside their own body, and the individual can only be restored with a successful exorcism and then Restoration.
If the host’s body is killed, there is a 25% chance that the hezrou demon drags the soul back to the Abyss with it. Otherwise it becomes trapped on the ethereal plane until such time as it can be rescued.
Blacktooth devourers are not very strong, but incredibly fast, nimble and vicious. A normal human cannot outrun a Devourer. They obey only the hezrou cultist who possesses their binding fetish.
They can be instructed to perform simple tasks, ("Guard this door", "Stay here and make sure he doesn't leave") but in the absence of any instruction, will simply attempt to kill and feed on any and every living creature that is not the fetish holder.
Lair and Lair Actions
The Blacktooth Devourer is a created creature, and as such, does not have a lair per se. Rather, they are kept in cells or pens, fed with live prisoners and any creatures that can be captured to sate their ravenous hunger.
