Traits

Acidic Skin. The Horror's skin is covered in an acidic gel. A creature that touches the horror or hits it with a melee attack while within 5 feet of it takes 7 (2d6) fire damage.

 

Actions

Bile Spray. The horror sprays bile from it's mouth in a 15 ft cone. All creatures in the attack must make a DC 16 Constitution saving throw or take 2d12 poison damage, or half as much on a successful save. 

Bite. Melee Weapon Attack: +8 to hit, reach 5 ft, 1 target. Hit: 13 (1d10 + 8) piercing damage.  If the target is a creature, it is grappled (escape DC 16). Until this grapple ends, the target is restrained, and the horror can't bite another target.

Swallow. The horror makes one bite attack against a Medium or smaller creature it is grappling. If the attack hits, that creature takes the bite's damage and is swallowed, and the grapple ends. While swallowed, the creature is blinded and restrained, it has total cover against attacks and other effects outside the horror, and it takes 10 (3d6) acid damage at the start of each of the horror's turns.

If the horror takes 25 damage or more on a single turn from a creature inside it, the horror must succeed on a DC 15 Constitution saving throw at the end of that turn or regurgitate all swallowed creatures, which fall prone in a space within 10 feet of the horror. If the horror dies, a swallowed creature is no longer restrained by it and can escape from the corpse using 15 feet of movement, exiting prone.

 

Description

The Anelid Horror is a large, pale worm that burrows through underground tunnels, sometime of it's own creation, hunting all manner of creatures. 

It lacks a true face, and instead, has just a toothy maw of several rows of circular teeth that chew and tear in opposite directions. The mouth is surrounded by 4 arms appearing vaguely humanoid. The horror is so pale and nearly transparent, that dark blood can occasionally be seen under its skin given enough light.
