Death's Despair. If at any point the Screamer is reduced to 0 hit points, it releases a deathly wail that pierces the mind of every creature within 20 feet. Each creature must succeed a DC 18 Wisdom saving throw or take 11 (2d10) psychic damage. On a success, a creature takes half damage.
Multiattack. The Screamer can use its Piercing Scream. It then makes 2 attacks with its handaxes.
Handaxe. Melee Weapon Attack: +3 to hit, reach 10 ft., one target. Hit: 6 (1d6 + 3) slashing damage.
Piercing Scream (Recharge 5-6). Each creature within 10 feet of the Screamer that can hear must make a DC 16 Wisdom saving throw or take 10 (2d8 + 2) psychic damage and become frightened for one minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If a creature’s saving throw is successful or the effect ends for it, the creature is immune to the screamer's Piercing Scream for the next 24 hours.
