Carrying a mortal host that can be released for a surprize attack.

 

Actions

Tentacle Lash. Melee Weapon Attack: +2 to hit, reach 15 ft., one target. Hit: 1d4 + 2 bludgeoning damage

Poison Dart. The void leech fires poison goo from the tip of his tentacles. The target has to make a DC 13 Constitution saving throw. On success it recieves 1d4+4 poison damage. On failure it recieves 1d4 poison damage on each turn for 1 minute. The effect ends with the death of the leech or if the target recieves heailng through a healing spell.

Cloud of Darkness. The void leech releases a cloud of darkness for 1 turn that blinds every entity in a radius of 20 ft. Every player has to throw a DC 13 dexterity saving throw. If it succeeds the cloud does not take affect. Failure: All attack rolls, roll with disadvantage. Recharge: The leech rolls 1d6 at the beginning of its turn. If it makes a 6 roll the ability is recharged and can be used on the same turn.

Gravitas. Once per day the void leech can concentrate gathered Aether to form a gravitational anomaly with it at its center. The spell is cast for one turn in which the leech cannot take any other action. In the next turn it unleashes an area of effect gravitation spell that causes 2d6+4 in a 10 ft. area of force damage. 

Bonus Actions

Devouer. The void leech can attack a target within a range of 15 ft (Mele attack  +4 to hit, one target). and burrow its tentacles into the target and transfer 1d4 HP onto itself.

 

Reactions

Spell Reflection. If the void leech makes a successful saving throw against a spell, or a spell attack misses it, the void leech can choose another creature (including the spellcaster) it can sense within 30 feet of it. The spell targets the chosen creature instead of the void leech. If the spell forced a saving throw, the chosen creature makes its own save. If the spell was an attack, the attack roll is rerolled against the chosen creature.

Description

This Monster resembles a mutated version of a Morbol. Since it comes from the 13th Shard, The World of Darkness also known as the Void, its main goal is to consume and gather aether.

The leech itself is a plump creature just like the Morbol but there has been some evolution through time. It is in a symbiotic or parasitic relationship with a host.

The leech imposes itself on a host. The unfortunate creature is most likely a mortal. This mortal gets sucked dry of body fluids by thousands upon thousands of teeth on the inside of the creature. The leech replaces them with a highly efficient, dark purple poison.

As long as the host is attached to the leech, it is immune to the poison.

Some of the teeth are longer and barbed. They connect to the nervous system of the host, robbing it of any kind of self-control. They get stuck and allow the leech to control the host remotely. The control is not perfect though, as it is dependent on the distance between leech and the host.

The host lives as long as the leech lives and can survive outside of the leech for a short while before succumbing to the poison.

The host provides cognitive abilities and intelligence to the leech aside from naturally generated aether that is refilled from the aether realm into the corporeal realm.

The most favoured hosts are mortals who are highly  intelligent and knowledgeable. The hosts on the other hand, as long as they manage to keep their sanity, are immortal. This Immortality is caused by the poison of the leech. Only few seek this kind of bond. Those who do, are resentful spirits of a world that is long gone.
