Anomalous Nature. The Plague Doctor does not need to eat or drink. 

Doctor's Bag. The Plague Doctor carries a black leather bag everywhere he goes. For all intents and purposes, this bag acts as a Bag of Holding, with its only known contents being a modified set of alchemist's supplies and an herbalism kit

Re-Animation. The Plague Doctor can use the corpse of a creature that it has killed with his Touch of Death attack to create an undead minion. He makes a medicine, and with a DC 16 success, he takes 4 hours to do so, using his alchemist's supplies and herbalism kit. He takes 8 hours instead if he fails the check. Use the zombie stat block for humanoid creatures, and use the matching stats of whatever animal was turned in this way if it is another creature, with the following changes:

  • The creature type is now undead.
  • Any attempt to re-animate a corpse higher than CR 4 automatically fails.

In combat, these zombies take their own initiative, and follow the orders of the Plague Doctor, requiring no action.

While the Plague Doctor claims that he is working on a perfect cure for the "Pestilence," his "cure" is far from perfect. The corpses that he uses in this feature do not remain forever. At the beginning of each day, the zombies must make a DC 13 CON saving throw. On a fail, the zombie dies and the corpse becomes immune to this feature permanently.

The Plague Doctor can have up to 5 minions at a time created in this way.

Pride In Your Work. The Plague Doctor's speed increases by 5 feet for each minion within 30 feet of him at the start of his turn

Actions

Multiattack. The Plague Doctor makes two Cane attacks.

Cane. Melee Weapon Attack: +4 to hit, reach 5 ft, one target. Hit: 10 (4d4) bludgeoning damage

Poison Syringe. The Plague Doctor throws a syringe at the nearest enemy creature with perfect accuracy. The syringe injects its payload on impact. The target must make a DC 15 CON save or become poisoned

Touch of Death. The Plague Doctor makes a melee spell attack aimed at the throat of the target creature (DC 17). The target must succeed a CON save or take 3d12 necrotic damage and become incapacitated. If the target succeeds, they take half damage and do not become incapacitated.

Bonus Actions

Disengage. The Plague Doctor can use the Disengage action as a bonus action

Description

The Plague Doctor was discovered after a series of missing person reports were filed in the area. He claims to be the only one who can cure a disease that only he can identify, killing those that are "infected" and augmenting their corpses to bring them back to life as mindless versions of themselves. He calls this disease the "Pestilence." If the Plague Doctor senses the Pestilence in a creature, humanoid or not, he is immediately hostile toward it.

Outside of searching for a cure, the masked physician tends to enjoy conversing with other scientists, often showing great interest in learning about some of the new medical procedures that have come about since he was first discovered.
