Traits

Conductor. Whenever the eel is subjected to lightning damage, it takes no damage and instead regains a number of hit points equal to the lightning damage dealt.

Hold Breath. The eel can hold it's breath for 1 hour.

Underwater Camouflage. The eel has advantage on Dexterity (Stealth) checks made while underwater.

Ever Alert. Due to the eel's multiple heads, it cannot be surprised and gains a +5 bonus to its initiative rolls.

Venom Glands. Some species of bloated eel have developed potent poison that they are able to excrete from their mouth and skin. These eels deal an additional 7 (2d6) poison damage on their main mouth bite attack, and creatures that make physical contact with the eel take 3 (1d6) poison damage.

Devoted Rearing. Bloated eels keep their eggs in specialized pouches in their mouths. They remain there for several months until the eggs hatch.

Actions

Multiattack. The eel makes four melee attacks, one bite attack (main mouth), two bite attacks (secondary mouths), and two claw or tentacle attacks. Any single attack can be replaced with a fling attack.

Bite (Main Mouth). Melee Weapon Attack: +12 to hit, reach 5 ft., one target. Hit: 22 (3d10 + 6) piercing damage.

Bite (Secondary Mouths). Melee Weapon Attack: +12 to hit, reach 10 ft., one target. Hit: 13 (3d4 + 6) piercing damage.

Claw. Melee Weapon Attack: +12 to hit, reach 10 ft., one target. Hit: 13 (3d4 + 6) piercing damage.

Tentacle. Melee Weapon Attack: +12 to hit, reach 20 ft., one target. Hit: 16 (3d6 + 6) bludgeoning damage, and the target is grappled (DC 16). Until this grapple ends, the target is restrained. The eel has six tentacles, each of which can grapple one target.

Fling. One large or smaller object held or creature grappled by the eel is thrown up to 20 feet in a random direction and knocked prone. If thrown target strikes solid surface, the target takes 3 (1d6) bludgeoning damage for every 10 feet it was thrown. If thrown at another creature, that creature must succeed on a DC 16 Dexterity saving throw or take the same damage and be knocked prone.
