Curse of Seeing. Azalphom receives an additional 3 bonus actions per round.

Spellcasting. The Fateweaver is a 20th-Level spellcaster. It has eradicated all early spell slots in exchange for more later spell slots. Its spellcasting ability is Wis (Spell Save DC 24, +15 to hit with spell attacks). Azalphom has the following spells prepared: 
6th Level (6 Slots): Harm, Heal
7th Level (5 Slots): Finger of Death, Project Image
8th Level (4 Slots): Feeblemind, Maze
9th Level (4 slots) : Foresight, Weird

Actions

Fate Rush. Azalphom may use an action to give disadvantage to a target who is making death saving throws.  

Dimensional Tendril. Melee attack, +9 to hit, reach 20ft., 2 targets. Hit: 38 (2d20 + 10) Slashing damage. 

Horrifying Eye. Ranged attack, +7 to hit, range 30/100 ft., 1 target. Hit: 41 (4d10 + 10) Radiant damage, Hit: 41 (4d10+10) Necrotic damage.

Peer into Nothingness. Ranged attack, +9 to hit, range 30/300ft., 1 target, Hit: 65 (8d10 +12) Psychic damage. Target must make a Wis saving throw of 18 or be frightened for 1 minute. 

Reactions

Rend from Record.   As a reaction to a melee attack, Azalphom may force the melee attacker to make a Wis saving throw of 18. If they fail, they take 6d8 Necrotic damage, and half as much on a successful save.

Purge from Power.  As a reaction to a spell attack, Azalphom may force the spellcaster to make an Int saving throw of 18. If they fail, they take 6d8 Radiant damage, and half as much on a successful save.

Strike from Safety. As a reaction to a ranged attack, Azalphom may force the ranged attacker to make a Cha saving through of 18. If they fail, they take 6d8 Psychic damage, and half as much on a successful save.

Legendary Actions

Fate Sealer. At the end of each turn, as long as it is not restrained, Azalphom selects a creature it can see and rolls a D20. If it rolls a 19 or a 20, that creature's soul is captured, and that player must roll both a Con and Wis saving throw of 15 or higher. If it fails one of those rolls, the player loses all of its remaining health and must start making death saving throws. If it fails both, the player is immediately dead. If it fails none, it loses half of its remaining health, rounded up.

Description

Azalphom, the Fateweaver is a gargantuan aberration, constructing of tendrils of shadow in a writhing mass. Its body is snakelike in form, and it extends beyond itself in wisps of shadow. Its head is an amalgamation of eyes shifting between shade and light in rapid succession.

The Fateweaver is as old as time, and is know as Mother of Reapers. It was imprisoned for throwing off the balance between good and evil, as it attempted to eliminated all life.

Lair and Lair Actions

Azalphom was trapped at the base of the Unending Stair, in a lair known as the Endthrone. Once the Scions of Shade were defeated in combat, and the Eyes of Losing destroyed, it was able to release itself from its possession within the throne. The Endthrone is a portal to rebirth, a powerful and massive artifact that acts as a portal between planes of life and death. Azalphom acted as a clog in this portal.
