Traits

Lightning Absorption. Whenever an astral shambling mound saplings is subjected to lightning or radiant damage, it takes no damage and regains a number of hit points equal to the damage dealt. If the hit points it regains would exceed its maximum, add the surplus to its Matter Pool.

Illumination. While it has at least 1 hit point, the legendary astral shambling mound sheds dim light in a 5-foot radius from its form.

Matter Pool.  Any non-radiant damage that the astral shambling mound saplings does to creature made out of organic matter adds to this pool. Once this pool equals the Saplings starting health it cannot add any more to the pool.

Swarming. A astral shambling mound sapling will move with the target it is attached to, if it is moving, and the target's movement speed is reduced as if it is dragging/carrying another small creature, unless the target is two or more sizes bigger than the Sapling.

If more than one Sapling attaches itself to a target then the target becomes grappled and must remove the saplings for it to be no longer grappled. A creature within 5 ft, including the target, can use its action to detach all saplings with a Strength Check (DC 13).

A sapling can detach itself by spending 5 feet of its movement. It can only do so after its Matter Pool is full and equals the amount of its staring hit points.

Actions

Piercing Slam. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 10 (2d6 + 6) [piercing] damage and the astral shambling mound sapling attaches itself to the target. While attached, the sapling doesn't attack. Instead, at the start of each of the sapling's turns, it will use the Gnaw action.

Gnaw. The astral shambling mound saplings gnaws on a creature that it has attached itself too. The attached target at the start of each of the Sapling's turns takes 6 (1d8+2) [piercing] damage and 6 (1d8+2 ) [radiant] damage. The sapling can only gnaw one creature at a time.

 

Bonus Actions

Sacrifice.  As a bonus action, if the astral shambling mound sapling is within 5 ft of another sapling, or the legendary astral shambling mound, it can add its Matter Pool amount as temporary HP to its target. Once this is done the Sapling is killed as it has merged with its targeted ally.

Description

A small, spikey plant creature with long prehensile tendrils and a root structure capable of moving itself and lunging at enemies. These saplings will seek out the closest organic matter and will lunge at its target with tendrils erect and ready to attach.  Its goal is to latch on to its prey and consume as much as it can.  Once that task is complete it returns to the legendary astral shambling mound to feed its creator and repair it. If a sapling is next to death and near another sapling, it will try to merge with it to allow the chance that the organic matter it has collected can make it back to its creator. The sapling was flung from a legendary astral shambling mound and acts as its ally.
