Foul. Any creature that starts its turn within 10 feet of the beast must succeed on a DC 18 Constitution saving throw or be poisoned until the start of the creature's next turn.
Regeneration. The Beast of Nurgle regains 20 hit points at the start of its turn. If it takes fire or radiant damage, this trait doesn't function at the start of its next turn. The beast dies only if it starts its turn with 0 hit points and doesn't regenerate.
Devour. Melee Weapon Attack: +13 to hit, reach 10 ft., one target. Hit: 17 (2d8 + 8) piercing damage. If the target is a Large or smaller creature, it must succeed on a DC 18 Dexterity saving throw or be swallowed by the worm. A swallowed creature is blinded and restrained, it has total cover against attacks and other effects outside the beast, and it takes 17 (5d6) acid damage at the start of each of the worm's turns.
If the beast takes 30 damage or more on a single turn from a creature inside it, the beast must succeed on a DC 20 Constitution saving throw at the end of that turn or regurgitate all swallowed creatures, which fall prone in a space within 10 feet of the worm. If the beast dies, a swallowed creature is no longer restrained by it and can escape from the corpse by using 20 feet of movement, exiting prone.
Slam. Melee Weapon Attack: +13 to hit, reach 5 ft., one target. Hit: 17 (2d8+8) bludgeoning damage.
Eject Slime (Recharge 5–6). The Beast of Nurgle spews out a corrosive slime, targeting one creature that it can see within 60 feet of it. The target must succeed on a DC 18 Dexterity saving throw or take 22 (4d10) acid damage.
