Legendary Resistance (5/Day). If the Avatar of Death fails a saving throw, it can choose to succeed instead.

Limited Magic Immunity. Unless it wishes to be affected, the Avatar of Death is immune to spells of 5th level or lower. It has advantage on saving throws against all other spells and magical effects.

Magic Weapons. The Avatar of Death's weapon attacks are magical.

Death Mark. When the Avatar of Death hits a creature with a melee weapon attack the creature gains a a Death Mark. A creature that has three Death Marks dies instantly and can not be resurrected by anything other than a wish spell. If a creature is under the influence of a spell or effect that prevents death, the effect is dispelled and the creature loses one Death Mark. Each Death Mark fades from a creature after 8 hours or if a lesser restoration spell is cast on the effected creature.

Spellcasting. The Avatar of Death is an 11th level spellcaster. Its spellcasting ability is INT (spell save DC 17, +9 to hit with spell attacks). The Avatar of Death has the following spells prepared:
Cantrips (at will): Toll the Dead
1st level (4 slots): Silvery Barbs, Bless

2nd level (4 slots): Spiritual Weapon, Blur

3rd level (4 slots): Animate Dead, Fear, Counterspell

4th level (3 slots): Greater Invisibility 

5th level (2 slots): Danse Macabre

Undead Thralls. Whenever the Avatar of Death creates an undead using a necromancy spell, it has additional benefits:

  • The creature's hit point maximum is increased by 20
  • The creature adds your +4 to its weapon damage rolls.
Actions

Multiattack. The Avatar of Death can make three attacks, using any combination of Harvester and Toll the Dead.

Harvester. Melee Weapon Attack: +9 to hit, reach 10 ft., 1 target. Hit: 2d12 + 5 slashing damage and an additional 1d12 necrotic damage.

Toll the Dead. Ranged Cantrip DC 17, range 120 ft., 1 target. Hit:  (3d10 + 5) necrotic damage.If the target is missing any of its hit points, it instead takes 3d12 + 5 necrotic damage.

Sure Shot. Melee Weapon Attack: guaranteed hit, reach 10 ft., 1 target. Hit: 2d12 + 5 slashing damage. Can not be used as part of a Multiattack.

Bonus Actions

Phantom Form. As a bonus action the Avatar of Death turns into a formless shadow that travels along the ground. While in this form the Avatar of Death can not be the target of attacks, spells, or abilities casted by other creatures, can not use it's flying speed, can not jump, can occupy the space of other creatures, and can not make attacks or cast harmful spells. As a free action the Avatar can revert back to it's original form.

Legendary Actions

The Avatar of Death has 3 legendary actions.

Teleport (Costs 2 Actions). The Avatar of Death teleports 120 feet in any direction.

Phantom Slide (Costs 1 Action). The Avatar of Death uses it's Phantom Form bonus action and teleports to an open space within 5 feet of a creature it can see. 

Cast a Spell (Costs 3 Legendary Actions). The Avatar of Death can cast any spell it has prepared, provided it has the spell slots to cast it.
