Poisonous Blood. Any creature that hits the Blood-starved Beast with a melee weapon or a natural weapon must succeed on a DC 15 Constitution saving throw or be poisoned for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Frenzied Malformation (Recharges after a Short or Long Rest). As it takes damage, the Blood-starved Beast's malnourished body twists and distorts further, granting it increased power and resilience.

The following changes occur when the Blood-starved Beast is reduced to 2/3 of its maximum hit points:

  • The Blood-starved Beast's hit point maximum increases by 60.
  • The Blood-starved Beast gains an additional claw attack when making a multiattack.
  • The Blood-starved Beast's movements become more erratic and unpredictable. Its reach increases to 15 feet for its claw attacks.

The following changes occur when the Blood-starved Beast is reduced to 1/3 of its maximum hit points:

  • The Blood-starved Beast enters a frenzied state. Its speed increases to 60 feet, and it gains the ability to use Toxic Spew as a bonus action, in addition to its regular actions.
  • The Blood-starved Beast's poison-infused attacks become even deadlier, causing the poisoned condition on a failed saving throw even if the target is immune to poison damage.
Actions

Multiattack. The Blood-starved Beast makes three attacks: two with its claws and one with its bite.

Claw. Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 14 (2d8 + 4) slashing damage.

Bite. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 18 (3d8 + 4) piercing damage plus 9 (2d8) poison damage. The target must succeed on a DC 15 Constitution saving throw or be poisoned for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Legendary Actions

The Blood-starved Beast can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The Blood-starved Beast regains spent legendary actions at the start of its turn.

  • Claw Attack. The Blood-starved Beast makes one claw attack.
  • Toxic Spew (Costs 2 Actions). The Blood-starved Beast expels a toxic cloud in a 20-foot radius centered on itself. Each creature in that area must make a DC 15 Constitution saving throw, taking 21 (6d6) poison damage on a failed save, or half as much damage on a successful one.
  • Quick Movement. The Blood-starved Beast moves up to its speed without provoking opportunity attacks.

Description

The Blood-starved Beast is a grotesque and malnourished monstrosity that haunts the streets of Yharnam. Its flesh hangs loosely from its skeletal frame, revealing its emaciated and starved form. The creature's body is covered in lesions and open sores, oozing with a toxic substance that seeps into the air, filling the surroundings with a putrid stench.

Once a person subjected to blood healing rituals, the Blood-starved Beast now embodies the consequences of such practices gone awry. Its connection to the Healing Church and the ritualistic blood administration is evident in its appearance and the presence of ritual blood nearby. The creature is believed to have been a Blood Saint, one of the chosen sources of healing blood, whose skin was mercilessly stripped to harvest its life-giving essence.

In battle, the Blood-starved Beast displays erratic and nimble movements, making it a formidable adversary. It relies on quick and aggressive frontal attacks, using its elongated claws and razor-sharp teeth to tear through its prey. Its strikes not only inflict devastating physical damage but also poison those unfortunate enough to be wounded by its toxic-laden claws and venomous bite.

Facing the Blood-starved Beast requires both agility and caution. Hunters must be vigilant, avoiding its toxic strikes while seeking openings to strike back. Antidotes prove crucial in countering its poisonous aura, ensuring survival in the face of this ferocious and diseased predator.
