Traits

Omni-Directional Eyes. The ankheg can't be flanked.

Acid for Blood. When the ankheg is hit with a melee attack, its carapace cracks open, and acid sprays the attacker. The creature who attacked the ankheg takes 7 (2d6) acid damage. Once the ankheg uses Acid for Blood, it can't do so again until it is at full hit points again.

Death Throes. When the ankheg dies, acid sprays each creature within 10 feet of it. Each creature in that area must make a Dexterity saving throw (DC 13), taking 10 (3d6) acid damage on a failed save, or half as much on a successful one.

Actions

Multiattack. The ankheg makes two attacks with its claws. When it does, they have a reach of 10 feet instead of 5 feet.
Alternatively, it can make two attacks with its claws. If both attacks hit, the ankheg can make a bite attack against the target of the claw attacks or push the target 5 feet away and knock them prone.

Bite. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 10 (2d6 + 3) slashing damage plus 3 (1d6) acid damage. If the target is a Large or smaller creature, it is grappled (escape DC 13). Until this grapple ends, the ankheg can bite only the grappled creature and has advantage on attack rolls to do so.

Claws. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 5 (1d4 + 3) slashing damage.

Acid Spray (Recharge 6). The ankheg spits acid in a line that is 30 feet long and 5 feet wide, provided that it has no creature grappled. Each creature in that line must make a DC 13 Dexterity saving throw, taking 10 (3d6) acid damage on a failed save, or half as much damage on a successful one.

Bonus Actions

Opportunistic Claw. The ankheg makes one attack with its claws against a creature that is prone and within the reach of its claws.
