False Appearance. While the worm remains motionless, it is indistinguishable from a normal flower. When disguised, and a target moves within 5ft of the worm, the worm releases a disgusting stench. All creatures within 5ft of the worm must succeed on a DC 15 Constitution save or fall unconscious. The target can repeat the save at the end of each of its turns.
Slam. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: 14 (3d6 + 4) bludgeoning damage.
Swallow. The Bloated Death Worm engulfs an unconscious target within 5ft of it. The target is blinded while inside the worm. If the worm takes 30 damage from a swallowed creature in a single turn then it must succeed on a DC 15 Constitution save or drop all swallowed creatures within 5ft of itself. If the Worm dies, all swallowed creatures can escape the worm by using 5ft of movement.
Description
Those who stray off boat paths in swamps may spot large purple and white flowers with intriguing patterns. Nearing these flowers will cause them to open up and reveal a large circular maw, before releasing a horrific, paralysis inducing stench. The flower itself is actually the evolutionary head of a large worm, which then swallows up its helpless prey and descends back into the ground.
