Traits

Bloody False Death (humanoid form only). On being reduced to 0 hit points while in humanoid form, the blood-plague pestilant explodes in a shower of blood causing 16 (3d10) necrotic damage to all creatures within 10 feet. A Dexterity saving throw reduces the damage by half. A creature that takes damage from this must make a DC 14 Constitution saving throw or become infected with the blood-plague disease (see main description).

After this explosion the remains of the blood-plague pestilant form an oozing mass of blood and part-liquefied organs that occupy the same space. It is healed of all damage and it's type changes to ooze. Other statistics remain the same but it's attacks change as noted under Actions. When reduced to 0 hit points in it's ooze form, the blood-plague pestilant is destroyed.

Curative Vulnerability. A Lesser Restoration spell cast upon the blood-plague pestilant causes 4d8 damage, while a Greater Restoration spell causes 12d8 damage (no save). In either case, this suppresses the blood-plague pestilant's Bloody False Death trait until the start of the caster's next turn: If it is reduced to 0 hit points within this time it is destroyed and does not transform into its ooze form.

Damage Resistances: Necrotic, Poison

Condition Immunities: Poisoned

Actions

Multiattack. The blood-plague pestilant makes two slam attacks (humanoid form only), or two pseudo-pod or blood squeeze attacks (ooze form only).

Slam (humanoid form only). Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit:  6 (1d8 + 2) bludgeoning damage.

Pseudo-pod (ooze form only). Melee Weapon Attack: +5 to hit, reach 10 ft., one target. Hit:  15 (3d8 + 2) bludgeoning damage and 3 (1d6) necrotic damage and the target is grappled. The blood-plague pestilant may grapple up to two targets this way.

Infectious Squeeze (ooze form only). Melee Weapon Attack: +5 to hit, one grappled target. Hit:  15 (3d8 + 2) bludgeoning damage and 3 (1d6) necrotic damage and the target must succeed on a DC 14 Constitution saving throw or become infected with the blood-plague disease (see main description).

Description

The blood-plague pestilant is, in reality, a supernatural demonic disease that typically festers in the most vile depths of the Abyss. There it lurks, waiting to infect any hapless mortal creature that happens to pass through the area of contagion and carry it back to the Material Plane. But it can also be brought in to the world through dark magic or evil corrupted artifacts, where it spreads as rapidly as it can.

The humanoid body of a blood-plague pestilant is swollen and disfigured, its skin almost completely covered in dark purple bruises and bleeding pustules. After it has transformed into its second ooze state, it appears as a horrific animated mass of bloody muscle and part-liquefied organs with lashing pseudo-pods of intestines and other bodily matter.

Blood-plague Disease

Born from the Abyss, this supernatural disease attempts to take control of its victim, transforming it into another blood-plague pestilant in order to spread.

An infected creature shows symptoms after 1d6 days. Symptoms include fatigue, and bleeding from the eyes and ears. The infected creature gains one level of exhaustion and any bludgeoning, piercing or slashing damage it receives is increased by 1d4 points per level of exhaustion.

At the end of each long rest, an infected creature must make a DC 14 Constitution saving throw. On a failed saving throw, the creature gains one level of exhaustion. On a successful save, the creature's exhaustion level decreases by one level. If a successful saving throw reduces the creature's level of exhaustion below 1, the creature recovers from the disease.

If a creature reaches 6 levels of exhaustion and dies, it's body is transformed into a blood-plague pestilant. This transformation robs the creature of it's mind and soul, and the newly born monster knows only mindless hate and rage, striking out at any one it can in the hopes of spreading the disease further.
