Insightful. The wolf has advantage on Wisdom (Perception) checks that rely on sight and it can’t be surprised.
Soul Drain. Whenever a creature still alive with 0 hit points is within 15 ft. of the wolf, it must succeed on a DC 16 Wisdom saving throw or die, its soul devoured. On a success, the creature is immune to Soul Drain for the next 24 hours.
Multi-attack. The wolf uses Invoke Beast if it can. It then makes three attacks: one with its bite and two with its claws.
Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 18 (3d8 + 4) piercing damage plus 4 (1d8) poison damage.
Claw. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 13 (2d8 + 4) slashing damage plus 4 (1d8) poison damage.
Invoke Beast (Recharge 6). The wolf’s many eyes focus their gazes on any number of creatures within 30 feet. Each target must succeed on a DC 16 Wisdom saving throw or become mad with beastial rage. The affected creature must attack the closest creature on their next turns or take 22 (4d10) psychic damage each time. An affected target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. A target that succeeds on the saving throw is immune to Invoke Beast for the next 24 hours.
Rampage. When the wolf reduces a creature to 0 hit points with a melee attack on its turn, the wolf can take a bonus action to move up to half its speed and make a bite attack.
Regenerate. The wolf regains 20 hit points at the start of its turn. Nothing will stop the regeneration while the wolf is alive, but if the wolf starts its turn with 0 hit points it will not regenerate and dies.
Wounded Fury. While it has 30 hit points or fewer, the wolf has advantage on attack rolls. In addition, it deals an extra 7 (2d6) damage to any target it hits with a melee attack.
Description
Normally the wolf spirit resides in those affected by lycanthropy, challenging the resident soul for control and tempting them to give in to their beastial natures. Those who give in to the curse become one with the wolf spirit. Those who try to remain civilised fight the internal struggle indefinitely. In this case, the wolf spirit has become increasingly aggressive; no doubt influenced by the Prince of Beasts, Baphomet, when the player characters disturbed his ancient shrine.
