Traits

Giant's Physiology. To attack any of the Titan's extremities, the player must be within reach and declare which body part they intend to attack. If the player misses, then the attack misses entirely. The body parts can all be divided into separate targets, each with its own Armor Class, Damage Threshold (for some of them) and Hit Points.

Head. AC: 13, HP: 9
Eye. AC: 15, HP: 1
Arm. AC: 11, HP: 12
Leg. AC: 8, HP: 15

The HP of the individual body parts is included in the HP maximum and any damage to a body part also affects the Titan's total HP.

For each arm the Titan loses, it also loses a claw attack. If one leg is severed, the Titan loses 10 ft. of movement and a claw attack, as it either loses an arm or must use an arm for movement. If it loses both legs, it falls prone. If it has both arms, it can crawl at half speed. With only one arm, it can still crawl, but its speed is 10 ft. With no arms or legs, its speed is 0, and it can’t benefit from bonuses to speed.

If both of the Titan's arms are severed, it loses its grapple attack. It can still do its bite and swallow attacks, but without having a creature grappled it does them with disadvantage.

If its head is severed, the Titan loses its bite attack and its body is blinded.

Regeneration. The Titan regains 2 hit points at the start of its turn. If the Titan takes acid or poison damage or spends the entire turn in complete darkness, this trait doesn't function at the start of the Titan's next turn. The Titan dies only if it starts its turn with 0 hit points and doesn't regenerate.

A severed body part regrows (and can be used) only if it regains all lost HP. It doesn't need to be at full HP to function after it has grown back, but once severed it must return to full before the Titan can use it again.

The regained HP are distributed randomly or at the DM's discretion.

Sunlight Immunity. The Titan is immune to all damage derived from sunlight both magical and non-magical.

Titan nature. A Titan doesn't require air or sleep. If it spends at least 30 minutes in direct sunlight it is considered to have consumed enough food and water to sustain it for 24 hours. If it spends 30 minutes out of sunlight, it becomes paralyzed until the sun hits it again.
A dawn spell, sunbeam spell or sunburst spell instantly brings any Titan in range up to full HP and regenerates all potentially lost limbs.

Actions

Multiattack. The Titan makes either three attacks: two with its claws and one grab, or a swallow/bite attack against a grappled creature.

Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 11 (2d6 + 4) piercing damage.

Claw. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 7 (1d6 + 4) slashing damage.

Grab. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 11 (2d6 + 4) bludgeoning damage. On a successful hit, the target is grappled (escape DC 14).

Swallow. If the Titan has managed to hold a target in a grapple for at least one round, the Titan can attempt to swallow the target. The Titan makes a bite attack against a target it has grappled. If the attack hits, the grapple ends and the target is swallowed. The swallowed target is restrained, it has total cover against attacks and other effects outside the Titan and takes 21 (6d6) acid damage at the start of each of the Titan's turns. The Titan can have one target swallowed at a time. If the Titan dies, any swallowed creature that is still alive is no longer restrained by it and can escape from the corpse using 5 feet of movement, exiting prone.

Reactions

Feed. Immediately after taking damage, if it has at least one creature swallowed, the Titan deals 10 (3d6) acid damage to each swallowed creature and regains the same amount of hit points.

Regurgitate. If all creatures within the Titan's stomach are dead, the Titan is forced to immediately throw them back up. This happens whether or not the Titan has already used its reaction that round, but costs one if it hasn't. Until the beginning of the Titan's next turn, all attacks against it are made with advantage.

Description

Though it shares its distorted and misgrown humanoid features with the Nerve Titans, the Blood Titan has skin that is constantly flushed red and mottled with bruising, and so these hulking giants look much more terrifying than their counterparts. They are, however, easier to kill since they can die from a thousand cuts and can be successfully attacked anywhere on their bodies. Legend has it that they used to be referred to as blood bags since you 'just' had to poke enough holes in them to drain and kill them, hence the name 'Blood Titan'.

A Blood Titan of this size contains approximately 15 litres of blood. When it is killed this blood covers the ground in a 10 ft. square where the Titan died. The ground becomes difficult terrain for the next round after which the blood evaporates.
