Nightmare Incarnate
If a creature starts its turn within 30 feet of the Bone Gnawer and can see the Bone Gnawer, the Bone Gnawer can force the creature to make a DC 12 Wisdom saving throw if the Bone Gnawer isn't incapacitated . On a fail, the creature becomes frightened . at the end of each of the creatures turns it can remake the save, ending the effect on successful save. If the target fails the saving throw by 5 or more, it is also paralyzed for the same duration. A target that succeeds on the saving throw is immune to the effects of the Bone Gnawer’s Nightmare Incarnate ability for the next 24 hours.
Multiattack. The Bone Gnawer makes one attack with its Bite, and one attack with its Needle Claws.
Bite. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 4) piercing damage.
Needle Claws. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 11 (2d6 + 4) piercing damage plus 10 (3d6) necrotic damage. If the target is a creature, it must succeed on a DC 12 Constitution saving throw or be cursed with a terrible hunger. The cursed target takes 10 (3d6) Necrotic damage for every 24 hours that elapse. Every day the target is cursed, it becomes increasingly mad, as an unnatural hunger for humanoid flesh and bones consumes it. If the creature spends at least a week with this curse left untreated, It’s madness can only be cured by a lesser restoration spell or other magic. If the curse reduces the target's hit point maximum to 0, the target dies, and its body turns into a new Bone Gnawer. The curse lasts until removed by the remove curse spell or other magic.
