Legendary Resistance (5/Day). If Ussarys fails a saving throw, he can choose to succeed instead.
Wrath of the Abyss Lord (Recharges after a Short or Long Rest). If Ussarys would be reduced to 0 hit points, his current hit point total instead resets to 500 hit points,he recharges his Calamity Breath and spell slots, and he regains any expended uses of Legendary Resistance. Additionally, Ussarys can now use the options in the "Mythic Actions" section for 1 hour.
Shroud of Darkness. Ussarys is shielded by an impenetrable shroud of darkness. While in this effect, he is protected from all attacks, spell effects, and damage. He cannot be targeted by an effect of any kind while this shroud is active.
His shroud is connected to a variety of servants. Upon these servants being defeated, the shroud is destroyed.
Innate Spellcasting. Ussarys can cast the following spells at will, requiring no components. His spellcasting ability is Charisma (Spell Save DC 26):
1/ Day: time ravage, blade of disaster, ravenous void, time stop
Multiattack. Ussarys can use Harrowing Presence. He then makes three attacks: one with his bite and two with his claws.
Bite. Melee Weapon Attack: +19 to hit, reach 15 ft., one target. Hit: 28 (8d6+10) piercing damage and 18 (6d6) necrotic damage.
Claw. Melee Weapon Attack: +19 to hit, range 15 ft., one target. Hit: 22 (4d12+10) slashing damage and 18 (6d6) necrotic damage.
Tail. Melee Weapon Attack: +19 to hit, reach 30 ft.one target. Hit: 28 (6d12 + 10) bludgeoning damage and 18 (6d6) necrotic damage..
Harrowing Presence. Each creature of the primordial dragon’s choice that is within 120 feet of him and aware of him must succeed on a DC 22 Wisdom saving throw or become frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
At the beginning of each frightened creature's turns, the creature takes 10 (2d10) necrotic damage and has its maximum hitpoints reduced by that amount. Ussarys regains missing hitpoints equal to the amount dealt.
Blackfire Breath. Ussarys exhales blackflame in a 30 ft. cone. All creatures in that cone must make a DC 26 Dexterity saving throw or take 7d10 fire damage and 7d10 necrotic damage, half on a success.
Calamity Breath (Recharge 5–6). Ussarys exhales a massive blast of energy in a 250 foot line that is 25 ft. wide. All creatures within the area must make a DC 26 Dexterity saving throw, or sustain 10d6 necrotic damage, 5d6 fire damage, 5d6 cold damage, 5d6 psychic damage, 5d6 force damage, and 5d6 acid damage, half on a successful save.
Shadow Fade. Ussarys vanishes, reappearing in an unoccupied space he can see within 120 ft.
Ussarys can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The spent actions are regained at the beginning of his turn
Attack. Ussarys makes one Claw or Tail attack.
Void Harrowing (Costs 2 Actions). Ussarys casts phantasmal killer, becoming invisible for the duration of the spell.
If the Wrath of the Abyss trait has activated in the last hour, he can use the options below as legendary actions.
Pandemonium (Costs 2 Actions). Ussarys targets a creature he can see within 120 ft. The creature must succeed on a DC 25 Charisma saving throw or take 44 (8d10) psychic damage and be hurled into a dark void, being held there and unable to escape. At the start of the aspect’s next turn, the creature reappears in an unoccupied space within 10 feet of Ussarys.
Devouring Void (Costs 3 Actions). Ussarys flares with energy. Each creature in a 60-foot-radius sphere centered on him must make a DC 27 Dexterity saving throw. On a failed save, the creature takes 39 (6d12) necrotic damage. Ussarys regains missing hitpoints equal to the total damage dealt. On a successful save, the creature takes half as much damage.
