Quantum Uncertainty. Any time the Quantum Phantom would be hit by an attack or affected by a magic spell that requires an attack roll, roll a d20. On an 18 or higher, the Phantom shifts slightly out of this dimension, causing the attack or spell to miss.
Multi-dimensional Existence. The Quantum Phantom can exist in multiple dimensions simultaneously. It cannot be restrained or trapped by any means, including magical effects.
Multiattack. The Quantum Phantom makes two phase touch attacks.
Phase Touch. Melee Weapon Attack: +10 to hit, reach 5 ft., one creature. Hit: 17 (3d8 + 5) necrotic damage. The target must succeed on a DC 17 Constitution saving throw or have its hit point maximum reduced by an amount equal to the damage taken. This reduction lasts until the target finishes a long rest. The target dies if this effect reduces its hit point maximum to 0.
Dimensional Flicker (Recharge 5–6). The Quantum Phantom randomly alters its position in the fabric of dimensions. Each creature within 30 feet of the Phantom must make a DC 17 Wisdom saving throw. On a failed save, a creature is teleported to a random location within 60 feet of its current position and is stunned until the end of its next turn. On a successful save, a creature isn't teleported or stunned.
Ethereal Stride. The Quantum Phantom teleports up to 60 feet to an unoccupied space it can see.
