Spectral Horde. Any creature that starts it's turn within 30ft of the death house must make a DC 13 wisdom saving throw or take 10 (3d6) psychic damage, and have their movement speed halved until the start of their next turn. 

Unstable Footing. The awakened death house has 5 overgrown fingers which it uses to maneuver around Barovia. Whenever the death house takes 25 or more damage in a single turn, one of its legs is destroyed. If only one of it's legs remains, it falls prone and cannot get up from prone. As long as the death house has more than three fingers to walk on, it can use it's multiattack action. 

Actions

Multiattack. As long as the death house has three or more legs, it can make two slam attacks with it's legs.

Slam. Melee Weapon Attack: +10 to hit, reach 10 ft., one target. Hit: 22 (3d10 + 6) bludgeoning damage, and the creature must make a DC 13 Strength saving throw or be grappled by the death house. 

Swallow. Melee Weapon Attack: +10 to hit, reach 10 ft., one target it is grappling. Hit: 33 (5d8 + 6) piercing damage, and the target is also swallowed, and the grapple ends. While swallowed, the target is restrained, it has total cover against attacks and other effects outside the death house. 

If the death house takes 30 damage or more on a single turn from the swallowed creature, the death house  succeed on a DC 15 Constitution saving throw at the end of that turn or regurgitate the creature, which falls prone in a space within 10 feet of the death house. If the death house dies, a swallowed creature is no longer restrained by it and can escape from the corpse by using 15 feet of movement, exiting prone.

Fade into Mist (Once per Dusk). The death house vanishes into the surrounding mist, and reappears anywhere it chooses within Barovia that is touching the mist. 

Reactions

Inescapable Fog. When a creature attempts to move more than 30ft away from the death house, the death house can force that creature to make a DC 13 Constitution saving throw, or be teleported to a point the death house chooses within 30ft. 

Description

Left behind to rot and decay within the death house, a portion of the dark god grew and grew, feeding and the pain and darkness trapped within the cursed mansion. Over time it spread throughout the house, and merged with the various souls that had died within it, creating a singular half-sentient entity. Now that the hand is large enough to even maneuver the house, it hunts for one of the few souls that were able to escape it, a soul known to few as Agent Orange.
