The Lich King is a 20th-level spellcaster. His spellcasting ability is Charisma (spell save DC 23, +15 to hit with spell attacks). He can cast spells from the anti-paladin spell list.

Unholy Smite: When you hit a creature with a melee weapon attack, you can expend one spell slot to deal necrotic damage to the target, in addition to the weapon's damage. The extra damage is 2d8 for a 1st-level spell slot, plus 1d8 for each spell level higher than 1st, to a maximum of 5d8. The damage increases by 1d8 if the target is a celestial or a fey, to a maximum of 6d8.

Actions
  • Multiattack: The Lich King can make three attacks with Frostmourne.
  • Frostmourne: Melee Weapon Attack: +16 to hit, reach 10 ft., one target. Hit: 13(2d8 + 11) slashing damage plus 9 (2d8) necrotic damage. The target must succeed on a DC 21 Constitution saving throw or its hit point maximum is reduced by an amount equal to the necrotic damage taken. This reduction lasts until the target finishes a long rest. The target dies if this effect reduces its hit point maximum to 0.
  • Death Coil (Recharge 5-6): Ranged Spell Attack: +16 to hit, range 120 ft., one target. Hit: 45 (10d8) necrotic damage, and the Lich King regains hit points equal to half the amount of necrotic damage dealt.
Legendary Actions

The Lich King can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. The Lich King regains spent legendary actions at the start of his turn.

  • Attack: The Lich King makes one attack with Frostmourne.
  • Frightening Gaze (Costs 2 Actions): The Lich King fixes his gaze on one creature he can see within 10 feet of him. The target must succeed on a DC 21 Wisdom saving throw or become frightened for 1 minute. The frightened target can repeat the saving throw at the end of each of its turns, with disadvantage if the Lich King is within line of sight, ending the effect on itself on a success.
  • Frost Aura (Costs 3 Actions): The Lich King releases a burst of icy energy. Each creature within 30 feet of him must make a DC 21 Constitution saving throw, taking 45 (10d8) cold damage on a failed save, or half as much damage on a successful one.

Lair and Lair Actions

The Lich King's Lair is the frozen throne atop Icecrown Citadel. On initiative count 20 (losing initiative ties), the Lich King takes a lair action to cause one of the following effects; the Lich King can’t use the same effect two rounds in a row:

  • Summon Undead: The Lich King summons 1d4 + 1 wights or 1d2 frost giants. The summoned creatures appear in unoccupied spaces within 60 feet of the Lich King and act as allies of the Lich King. They remain for 1 hour, until they or the Lich King dies, or until the Lich King dismisses them as a bonus action.
  • Frozen Ground: The ground in a 30-foot radius centered on a point within 120 feet of the Lich King becomes icy and difficult terrain for 1 minute.
  • Necrotic Burst: The Lich King releases a burst of necrotic energy. Each creature within 30 feet of him must make a DC 21 Constitution saving throw, taking 36 (8d8) necrotic damage on a failed save, or half as much damage on a successful one.
