Traits

 Mushroom: The dragon has advantage on stealth checks made while in a space with lots of mushrooms. (A field full of mushrooms, a cavern full of mushrooms, a mushroom garden, ect.)

Regrowth: The dragon heals for 1d12 at the end of it's turn unless if it is at zero hit points or if it took fire damage this round. Any lost limbs grow back after one week.

 

Actions

Multi-attack: The dragon makes two claw attacks and one bite attack.      

Bite: Melee Weapon Attack:  +10 to hit, reach 5 ft., 1 target. Hit: 12 (2d8 + 4) piercing damage and the target makes a Constitution saving throw 15 or takes 24 (6d8) poison damage.

Claw: Melee Weapon Attack:+10 to hit, reach 5 ft., 1 target. Hit: 12 (2d8  + 4) slashing damage.

Tail: Melee Weapon Attack:+10 to hit, reach 5 ft., 1 target. Hit: 12 (2d8  + 4) bludgeoning damage.

  Spore Breath (Recharge 5–6) The dragon breaths out a 40ft. cone of spores. Any hostile creature in that radius must make a Constitution saving throw 17 or take 36 (9d8) poison damage and become poisoned until the start of the dragon's next turn. When you start your turn poisoned in this way you take 36 (9d8) poison damage. If the hostile creature succeeds the saving throw they take half damage and aren't poisoned. Any creatures friendly towards the dragon heals for 9d8 and heals for 9d8 at the start of their next turn.

Dragon Spore Breath(1/Day): The dragon breaths spores in a 30ft. cone. Any creature in the cone must make a Constitution saving throw 16 or ingest a dragon spore. This spore rapidly turns into a mushroom dragon wyrmling. At the start of the players turn they take 4d6 poison damage. if they drop to zero hp they die and a mushroom dragon wyrmling comes up it's throat. This can be prevented by using a lesser restoration spell or similar magic. When the dragon uses this breath in an area with mushrooms 1d6 of those mushrooms die being replaced with mushroom dragon wyrmling in three rounds.

Shroom Bomb(3/Day): The dragon creates a large glowing mushroom that is 10ft. tall and 5ft. wide. It has 75hp and a AC of 15. At the end of the dragons next turn any create other than the dragon within 15ft of the mushroom must make a Dexterity saving throw 15 or take (6d6) force damage or half as much if they succeed because the mushroom exploded. They also must make a Constitution saving throw 15 or take (8d6) poison damage and become poisoned. If the dragon is within 15ft. of the mushroom he heals for 8d6.

 

Bonus Actions

Shape Change(4/Day): The dragon changes into a human form. While in this from it cannot make bite and claw attacks. If it is already in human from it changes back into a dragon.

Legendary Actions

The dragon has three legendary actions which it can take after a different creature ends it's turn. It regains the legendary actions at the start of it's turn.

Regeneration(Cost Two Actions)(3/Day): The dragon regains 6d6 hit points.

Toxic Shrooms(Cost Three Actions)(Can only be used if in an area with abundant mushrooms.): The shrooms in a 30ft. circle of the dragon turn into a deathly green and start emitting gas. Whenever a creature other than a mushroom dragon starts it's turn in the toxic shrooms it must make a Constitution saving throw 14 or take (3d8) poison damage and become poisoned until they leave the circle. If the dragon starts it's turn in the circle it regains 3d6 hp. This effect lasts for three rounds. After the three rounds the mushrooms die and regrow in 1d4 days as normal mushrooms. 

Tail: The dragon makes a tail attack.

Description

It is not known where these dragon came from. Many think it was a wizards pet project. Others think they are from a uncharted plane, the way in hidden deep in the Astral plane.

Whatever the case is, mushroom dragons are rather friendly for the most part. Most like a good chat or story over a good battle. During one of these chats a adventurer claims the dragons said they don't even know where they came from. This theory is still in question.

Mushroom dragons usually build there homes in groups until they migrate at adulthood. They make there homes in areas covered in mushrooms for the most part. Some choose otherwise. They are collectors of knowledge, always wanting to know more of the world. They usually get this knowledge from adventures who happen to cross paths with them. Only a few have ever ventured away from mushrooms to travel the world and collect the knowledge for themselves. Fewer still have come back. They eat mushrooms for the most part, making sure to replant them. They also enjoy to eat plants and the occasional meat.
