Phyrexian Anatomy. Whenever the surgeon takes piercing or slashing damage, one creature in the direction the damage came from must succeed on a DC 11 Constitution saving throw or gain 1 level of Phyresis.
Wire Flier. The atmosphere surgeon is connected to cables that attach to the ceiling. This collection of wires and tubes gives the surgeon its flying speed. These cables can be attacked (AC 15) and destroyed (20 HP, immune to poison and psychic damage), using the surgeon's saving throw bonuses. If these cables are destroyed, the surgeon loses its flying speed and falls, taking damage as normal.
Claw. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 5 (1d6 + 2) slashing damage plus 6 (1d12) lightning damage.
Atmospheric Surgery (Recharge 5-6). The surgeon suffuses the air in a 30-foot radius centered on a point it can see within 60 feet of it with toxic gas. Any creature in this area must make a DC 14 Constitution saving throw, taking 7 (2d6) necrotic damage plus 7 (2d6) poison damage on a failed save, or half as much damage on a successful save.
Spellcasting. The surgeon casts one of the following spells, requiring no material components and using Intelligence as the spellcasting ability (spell save DC 14):
