Aura of Despair: Each creature within 30 feet of the Voidborn Horror must make a DC 15 Wisdom saving throw. On a failed save, the creature is frightened until the end of its next turn.

Actions

 

Multiattack: The Voidborn Horror makes two attacks: one with its Ethereal Touch and one with its Entangle.

Entangle (1st-level spell): The Voidborn Horror causes grasping tentacles to sprout from the ground in a 20-foot square centered on a point it can see within range. Each creature in that area must succeed on a DC 16 or be restrained until the spell ends. A restrained creature can use its action to make a Strength check against the DC. On a success, it frees itself.

Ethereal Touch: Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 27 (6d8) necrotic damage. The target must succeed on a DC 15 Constitution saving throw or have its hit point maximum reduced by an amount equal to the necrotic damage taken until it finishes a long rest.

Disintegration Gaze (Recharge 5–6). The beast targets one creature it can see within 60 feet of itself. The target must make a DC 18 Constitution saving throw, taking 70 (10d6 + 40) force damage on a failed save, or half as much damage on a successful one. If this magical effect reduces the target to 0 hit points, the target is disintegrated and leaves nothing behind, except a pile of ashes plus whatever equipment it was wearing or carrying.

Bonus Actions

Ethereal Jaunt: As a bonus action, the Voidborn Horror can shift into the Ethereal Plane, allowing it to move through objects and creatures as if they were difficult terrain. It can remain in the Ethereal Plane for up to 1 minute before reappearing in its original location.

Legendary Actions

The beast can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. The beast regains spent legendary actions at the start of its turn.

Shadow Step. The Voidborn Horror moves up to half its speed without provoking opportunity attacks.

Horrifying Visage (Costs 2 Actions). Each creature of the Voidborn Horror's choice within 30 feet must succeed on a DC 16 or become frightened until the end of its next turn.

Ethereal Bolt (Costs 3 Actions). The Voidborn Horror unleashes a bolt of dark energy. Ranged Spell Attack: +8 to hit, range 120 ft., one target. Hit: 3d10 necrotic damage.

Description

The Voidborn Horror is a terrifying entity that emerges from the abyss, its form shifting and ever-changing. It appears as a mass of swirling darkness, punctuated by flickering points of cold, blue light that resemble distant stars. The air around it shimmers with an unsettling energy, creating an atmosphere of dread. As it moves, it leaves a trail of darkness, and the whispers of lost souls can be heard emanating from its depths. The creature radiates an aura of despair, feeding on the fears and nightmares of those who encounter it.
