Blighted Resilience. The weasel is resistant to blighted energy and partially immune to the Blighted Scourge. It can be infected and reach stage 4, but it will not transform into a Blighted creature upon death.

Illusionist's Cloak. The Unseen Phantom Weasel can cast the invisibility spell on itself at will, without expending a spell slot or material components.

Master of Deception: The Phantom Weasel has advantage on Charisma (Deception) checks and can cast the minor illusion cantrip at will, it can create complex illusions, up to six times normal size, that emit sound and faint movement.

Illusionary Escape (Recharge 4–6). The Unseen Phantom Weasel can use its reaction to cast mirror image or blur when targeted by an attack or spell.

Greater Unseen Illusions: The Phantom Weasel retains invisibility in dim light or darkness, undetectable by non-magical means, it can become visible in bright light but regains invisibility as a bonus action in dim light or darkness.

Phantasmal Shroud. The Phantom Weasel exudes an aura of illusory mist within 30 feet, causing all enemies to have disadvantage on Perception checks. Hostile creatures within the aura also have disadvantage on attack rolls against the Phantom Weasel.

Humanoid Shapeshift (2/Day). The Unseen Phantom Weasel can transform into a humanoid form for up to 2 hours. In this form:

  • It retains its Intelligence, Wisdom, and Charisma, but loses its natural weapons.
  • While in this form, its AC is 18.
  • It automatically reverts to its original form if it drops to 0 hit points or as a bonus action.

Claws. Melee Weapon Attack: +11 to hit, reach 5 ft., one target. Hit: 18 (3d8 + 6) slashing damage.

Phantasmal Assault (Recharge 5-6). The Phantom Weasel creates an illusory, monstrous creature within 60 ft. of a target. The target must succeed on a DC 18 Wisdom saving throw or take 24 (6d6 + 5) psychic damage and become charmed until the end of the Phantom Weasel's next turn.

Bonus Actions

Shadow Step. The Phantom Weasel can teleport up to 60 feet to an unoccupied space it can see in darkness or dim light.

Reactions

Cloak of Shadows (1/round). When attacked by a melee or ranged attack, the Unseen Weasel can use its reaction to create an illusory duplicate of itself, causing the attack to miss if it rolls a 15 or lower on a d20.

The Unseen Phantom Weasel exists somewhere between reality and illusion, a living mirage that can shift between shadow and form at will. Few who encounter one even realize they are being watched—a whisper in the dark, a shifting silhouette, a breath of movement too quick to grasp.

Whereas the lesser ranks of their kind rely on simple tricks, the Phantom Weasels can create entire false environments, distorting landscapes and turning familiar places into unrecognizable labyrinths. The illusions they craft are not merely sights and sounds but living, breathing manifestations of deception—so real that even the wisest struggle to unravel them.

Their Phantasmal Shroud ensures that even if someone manages to see through their illusions, their very presence is shrouded in a field of warped perception, making it nearly impossible to land a strike. This ability allows them to toy with their prey, slipping between shadow and substance with ghostly precision.

Even more astonishing is their ability to assume humanoid form for a time, seamlessly integrating into the world around them, observing, manipulating, and orchestrating events without ever being suspected.

A battle against an Unseen Phantom Weasel is never what it seems. By the time one realizes they are fighting, they have already lost.
