Traits

Ashen Form. The elemental can enter a hostile creature's space and stop there. It can move through a space as narrow as 1 inch wide without squeezing. A creature that starts its turn in a space that was occupied by the elemental at any point during its last turn must make a DC 14 Constitution saving throw, taking 11 (2d10) poison damage and being Poisoned on a failed save, or taking half as much damage and not being poisoned on a successful one.
   At the end of a creature's turn, if they are outside of an area affected by the elemental's Ashen Form or Cloud of Smog they can attempt the Constitution saving throw again, ending the poisoned condition on them on a success.

Harbringer of Plague. A creature poisoned by the elemental takes 5 (1d10) necrotic damage at the start of their turn and must succeed on a DC 14 Constitution saving throw or have their hit point maximum reduced by an amount equal to the necrotic damage they took. If a creature's hit point maximum is reduced to 0 as a result, they immediately die and their body starts turning into ash, becoming a pile of ash once a minute has passed. This reduction to a creature's hit point maximum ends when they finish a long rest.

Wind Susceptibility. If the elemental or any space it had occupied during its last turn is hit by strong enough wind, the damaging effects of its Ashen Form don't count for those spaces, and the elemental takes 6 points of force damage if it starts its turn within an area of such strong wind.

Actions

Multiattack. The elemental makes two slam attacks.

Slam. Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 9 (1d8 + 5) bludgeoning damage plus an additional 11 (2d10) poison damage.

Cloud of Smog (Recharge 5-6). The elemental unleashes noxious fumes into the area around it. Each creature in a 20-foot-radius sphere centered on the elemental must make a DC 14 Constitution saving throw. On a failed save, they take 16 (3d10) poison damage and are Poisoned. On a successful save, they only take half as much damage and aren't poisoned. At the end of a creature's turn, if they are outside of an area affected by the elemental's Ashen Form or Cloud of Smog they can attempt the Constitution saving throw again, ending the poisoned condition on them on a success.
   Until strong wind disperses it or until a minute passes, the noxious fumes roll out along the ground, moving 10 feet away from the center of the sphere at the start of each of the elementals turns. The fumes can't move through total cover.
