Blood Frenzy. The were-hammerhead shark has advantage on melee attack rolls against any creature that doesn't have all its hit points.

Shapechanger. The were-hammerhead shark can use its action to polymorph into a shark-humanoid hybrid or into a hammer head shark, or back into its true form, which is humanoid. Its statistics, other than its AC, are the same in each form. Any equipment it is wearing or carrying isn't transformed. It reverts to its true form if it dies.

Blood in the Water. The were-hammerhead shark has advantage on Wisdom (Perception) checks that rely on smell but only when a creature doesn't have all it's hit points.

Blind Spot. The Hammer Head Were-Shark has disadvantage when attacking a creature that is directly in front of it. This ability is not nullified by Blood Frenzy.

Actions

Multiattack. In shark form, The were-hammerhead shark makes two bite attacks. In humanoid form, it makes two Meat-Hook attacks. In hybrid form, it can attack like a shark or a humanoid.

Bite (Shark or Hybrid Form Only). Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 15 (2d10 + 4) piercing damage. If the target is a humanoid, it must succeed on a DC 14 Constitution saving throw or be cursed with were-hammerhead shark lycanthropy.

Description

Deep within the darkest seas, a goddess both terrible and beautiful placed a curse upon those men and women who travel her oceans without tribute. Said to be taken by the ocean during terrible storms and waves, ships torn asunder from their moorings when the goddess becomes displeased. Those not killed by the storms, sink to the bottom, and rise to meet their goddess as were-sharks.

Hammerhead Weresharks are a variant of wereshark that takes on the characteristics of native hammerhead sharks. Sly and cunning, they are apex predators of the sea, their prowess and cruelty unmatch, even among other weresharks, as they've been known to partake in cannibalism of other hammerhead weresharks. Typically a pirate, privateer or any other sailor would have this curse befall upon them, perhaps by an angry sea god or a sea-witch. Still others are cursed by the bite of a wereshark, forced to transform into wanton bloodthirsty predators at the peak of high tide.

Those weresharks that can withstand the cold calculating animal that lurks skin deep beneath them become pirates. Typically not industrious, they will join a pirates crew, and then whisper the idea of mutiny to the crew. Overthrowing the captain, the hammerhead were shark takes command and begins the process of pillaging and devouring all it may across the coast it haunts.
