Regeneration. Athaekeetha regains 20 hit points at the start of its turn if it has at least 1 hit point and isn’t in running water or exposed to bright sunlight. If it takes radiant damage or damage from holy water, this trait doesn’t function at the start of its next turn.
Magic Resistance. Athaekeetha has advantage on saving throws against spells and other magical effects.
Undead Nature. Athaekeetha doesn’t require air, food, drink, or sleep.
Multiattack. Athaekeetha makes four tentacle attacks.
Tentacle. Melee Weapon Attack: +9 to hit, reach 5 ft., one creature.
Hit: 10 (1d10 + 4) bludgeoning damage plus 14 (4d6) necrotic damage and the target must succeed on a DC 18 Constitution saving throw or gain 2 levels of exhaustion. For each level gained, Athaekeetha regains 10 hit points. The brain of a creature killed this way is left ripe for consumption. If the target is stunned, Athaekeetha automatically grapples it (escape DC 18).
Mind Blast (Recharge 5–6). Athaekeetha emits a psychic wave in a 60-foot cone. Each creature in the area must make a DC 18 Intelligence saving throw. On a failed save, the creature takes 27 (6d8) psychic damage and is stunned for 1 minute. A stunned creature can repeat the saving throw at the end of its turns, ending the effect on itself on a success. Creatures must also succeed on a DC 18 Wisdom saving throw or gain a short-term madness effect.
Consume Brain (1/Day). Athaekeetha attempts to extract the brain of a creature it has grappled. The creature must make a DC 18 Intelligence saving throw. On a failed save, the creature takes 55 (10d10) psychic damage. If this damage reduces the creature to 0 hit points, its brain is consumed, and it dies instantly.
Athaekeetha can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time, and only at the end of another creature’s turn. Athaekeetha regains spent legendary actions at the start of its turn.
Detect. Athaekeetha makes a Wisdom (Perception) check.
Tentacle Strike. Athaekeetha makes a tentacle attack.
Psionic Terror (Costs 2 Actions). Athaekeetha unleashes a burst of psychic fear. Each creature within 30 feet that can see Athaekeetha must succeed on a DC 18 Wisdom saving throw or be frightened for 1 minute. A frightened creature can repeat the saving throw at the end of its turns, ending the effect on itself on a success.
Description
Athaekeetha, the most infamous of the vampire illithids, is a terrifying aberration born of mad experiments. Its gaunt, rubbery form and elongated feeding tentacles suggest both its undead nature and its illithid heritage. Though it cannot communicate as its living kin do, it exudes an aura of dread and insanity.
Driven by a hunger for brains and life energy, Athaekeetha prowls the dark reaches of Ravenloft, spreading chaos and death. It despises sunlight and bright lights, preferring the misty borders of Bluetspur to sate its predatory appetites.
Lair and Lair Actions
If encountered in its lair, Athaekeetha can invoke its connection to the psionic energies of Bluetspur. On initiative count 20 (losing initiative ties), Athaekeetha can use one of the following lair actions:
- Mind Warp. Athaekeetha targets one creature within 120 feet. The target must make a DC 18 Intelligence saving throw or take 18 (4d8) psychic damage and suffer a long-term madness effect.
- Psionic Shield. Athaekeetha gains a +2 bonus to AC and saving throws until initiative count 20 on the next round.
- Dimensional Shift. The terrain within 60 feet of Athaekeetha briefly twists and reshapes. All creatures in the area must succeed on a DC 18 Dexterity saving throw or be restrained until the start of Athaekeetha's next turn.
