Ethereal Vision: Has Truesight for 300ft, however, if the Astral Leech decides to turn all of its appendages into mouths, it loses it's All Around Vision as well as it's Truesight and instead has Blindsight for 10ft

Actions

Multiattack: Makes a number of Bite attacks equal to the current number of mouths the creature has (1-3). It then uses an appendage that is currently an eye to make a Piercing Gaze attack (if it is able)

Bite: Melee Weapon Attack: +7 to hit, reach 5 ft., 1 target. Hit: # (2d8 + 3) [piercing] damage.

Piercing Gaze: Ranged spell: DC 15, range 60ft., 1 target in sight. Target makes a wisdom saving throw, on a fail, target takes: 2 (1d4 + 4) [psychic] damage, the next attack the Astral leech makes against that target has advantage. (This attack cannot be used if the Astral Leech has the blinded condition)

Bonus Actions

Change Appendages: The Leech changes it's composition of eyes and mouths between its 3 appendages.

Siphon Soul: When reducing a creature to 0 hit points, it can choose to devour it's astral energy and regain hit points equal to 2d8 + 2

Spell Absorption: Recharge 5-6

When being damaged by a spell, the Astral Leech can attempt to reduce the damage of the spell by absorbing its energy.

When attempting to absorb damage from a spell, reduce the damage by 2d10 + 4 and reduce the damage taken equal to the number rolled. If the roll reduces the damage to 0, it receives no damage and is treated as if it succeeded the saving throw or the attack missed (any spells that still would deal damage on a successful save deal no damage).

If the damage of a spell is reduced to 0, the Astral Leech can chose to do one of the following things:

  • Reflect the spell back at a target they can see within the range of the spell absorbed. The spell attack bonus or spell save DC is the same as when it was originally cast
  • Restore a number of hit points equal to half the damage absorbed
  • Gain resistance to the damage type of the damage absorbed for 1 minute, in which they take on an altered physical form akin to whatever damage was absorbed (cannot use this ability again until the current resistance wears off)

The Astral Leech will always use this ability when given the chance

Description

The Astral Leech is a large wriggly aberration hailing from the astral plane. Its body consists of a long worm-like body with a large singular large eye or mouth on the front end. Connected at points just below the "head" are two long prehensile appendages of a similar composition, each with either another large eye or mouth on the end. The Leech can shift its head and appendages between these two body parts freely with little effort, these parts are used for differing purposes and are thus subbed in or out as needed.

Their multiple prehensile appendages which can take the form of massive glassy eyes give them an extremely wide field of few, nearing 360 degrees of vision. Their massive field of view isn't the only interesting thing about their vision, their eyes are also trained to see through all forms of illusory magic and shapechanging, due to their perception being anchored in the ethereal and astral planes, planes which rely on feeling and thought rather than physical outward appearance

Astral Leeches are social by nature, roaming in small pods ranging from 3-7. Their intelligence also makes them inquisitive and curious when observing new things, but are extremely ferocious once they recognize something as potential prey. These creatures feed on astral energy, which includes things such as the matter the ethereal and astral planes, the energy of souls and thoughts, and the projections of magical energy that make up spells. When not idly grazing on the ethereal or astral matter of their home plane, they hunt in pods to chase down larger ethereal and astral creatures, with which they are known to latch onto and feed on for extended periods of time (much like real leeches), or occasionally caravans of wayward travelers lost in the endless fog of said planes.
