Traits

Relentless (Recharges after a Short or Long Rest): If the abyssal pig takes 10 damage or less that would reduce it to 0 hit points, it is reduced to 1 hit point instead.

Demonic Stench: Any creature that starts its turn within 5 feet of the abyssal pig must succeed on a DC 12 Constitution saving throw or be poisoned until the start of its next turn.

Corrupted Feast: When the abyssal pig reduces a creature to 0 hit points with a gore attack, it can use a bonus action to devour part of the creature. When it does so, it gains 5 1d8 + 1 temporary hit points.

Actions

Multiattack: The Abyssal Pig makes two attacks: one with its gore and one with its bite.

Gore: Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 3) piercing damage.

Bite: Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 6 (1d6 + 3) piercing damage. If the target is a creature, it must succeed on a DC 12 Constitution saving throw or become diseased. The diseased target can't regain hit points except by magical means, and the target's hit point maximum decreases by 3 (1d6) every 24 hours. If the target's hit point maximum drops to 0 as a result of this disease, the target dies. The disease lasts until removed by the lesser restoration spell or similar magic.

Hellish Squeal (Recharge 6): The abyssal pig releases a terrifying squeal in a 15-foot cone. Each creature in that area must succeed on a DC 12 Constitution saving throw or take 10 (3d6) thunder damage and be deafened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the deafened condition on itself on a success.

Description

This corrupted swine stands as tall as a human waist, its form a nightmarish twist on a normal pig. Its bristles are metallic and razor-sharp, occasionally dripping with a caustic ichor that sizzles when it hits the ground. The creature's tusks are oversized and seem to pulse with an unholy light, while its snout is split into three separate parts, each lined with rows of needle-like teeth.

Its hooves leave smoldering footprints that emit a sickly green vapor, and its skin appears to be constantly shifting and bubbling as if something writhes beneath it. The beast's empty eye sockets weep a black, tar-like substance that burns whatever it touches. When it opens its mouth to squeal, multiple tongues can be seen writhing inside, and the sound it produces seems to carry the weight of countless tortured souls.

The presence of an abyssal pig is often preceded by the smell of rotting flesh mixed with burning sulfur. They're known to hunt in small groups called "blights" and are particularly drawn to holy sites and consecrated ground, which they seem to take pleasure in defiling. Some demon worshippers keep them as pets, though even the most devoted cultists must take extreme precautions against their corrupting presence.
