Traits

Incorporeal Movement.
The Astral Wraith can move through other creatures and objects as if they were difficult terrain. It takes 5 (1d10) force damage if it ends its turn inside an object.

Mind-Warping Presence (Recharge 5–6).
Each creature within 30 feet of the Astral Wraith must make a DC 13 Wisdom saving throw or take 1d4 psychic damage and become frightened for 1 minute. A frightened creature may repeat the saving throw at the end of its turns, ending the effect on a success.

Astral Tether.
If an Astral Wraith is slain in the Astral Plane, its essence reforms in 1d6 days unless destroyed by radiant damage or an effect like Dispel Evil and Good.

Actions

Multiattack.
The Astral Wraith makes two Mind Drain attacks.

Mind Drain. Melee Spell Attack: +4 to hit, reach 5 ft., one creature.

  • Hit: 9 (2d6 + 2) psychic damage, and the target must make a DC 13 Intelligence saving throw.
    • On a failure, the target's Intelligence is reduced by 1d4 until it finishes a long rest.
    • If a creature’s Intelligence reaches 0, it falls unconscious until it regains at least 1 point of Intelligence.
Legendary Actions

Void Shift (Costs 1 Action).
The Astral Wraith momentarily vanishes into the void. Until the start of its next turn, it is invisible and immune to nonmagical damage.

Description

An Astral Wraith is the lingering essence of a soul lost in the Astral Plane, stripped of its mortal identity and reduced to a spectral predator. These entities drift silently through the silvery void, seeking to consume the thoughts and memories of the living.

Physical Description:

  • Formless and Wispy – Its body is an ethereal shroud of shifting stardust and shadow, appearing as a ghostly figure with jagged edges that fade into the void.
  • Glowing Eyes – Hollow sockets radiate a dim, eerie blue or violet glow, pulsing like distant dying stars.
  • Disjointed Movements – The wraith moves with unnatural fluidity, flickering across space as though reality itself cannot fully contain it.

Behavior & Abilities:

  • Mind Eaters – Instead of draining life, Astral Wraiths feed on intelligence, causing victims to suffer mental deterioration and confusion.
  • Frightening Presence – The unnatural nature of an Astral Wraith exerts a subtle yet terrifying pressure on those who encounter it, overwhelming weaker minds with existential dread.
  • Unmoored – As beings of the Astral Plane, they do not obey the physical laws of the Material Plane, hovering weightlessly and passing through objects with ease.

Lore & Origins:

  • Lost Souls – Some Astral Wraiths are the remnants of travelers who perished in the void, unable to find peace or passage to the afterlife.
  • Created by Catastrophe – When great tragedies occur in the Astral Sea, such as the destruction of a ship or the collapse of a portal, the souls of those caught in the disaster may be warped into these malevolent beings.
  • Bound to the Astral Plane – Unlike Material Plane wraiths, Astral Wraiths have no ties to undeath or necromancy; instead, they are echoes of consciousness unmoored from reality.
