Primordial Fear. Wisdom Saving throw: DC 21, any humanoid that starts its turn in a 30 ft. Emanation originating from the Angel of Death. Failure: The target is frightened until the end of its next turn.
Legendary Resistance (4/Day). If The Angel of Death fails a saving throw, it can choose to succeed instead.
Magic Resistance. The Angel of Death has Advantage on saving throws against spells and other magical effects.
Incorporeal Movement. The Angel of Death can move through other creatures and objects as if they were difficult terrain. It takes 5 (1d10) force damage if it ends its turn inside an object.
Touch of Death. When a creature fails a saving throw controlled by or are hit by an attack from The Angel of Death lose two unspent hit die, if none are unspent, it takes necrotic damage equal to one of its hit die and The Angel of Death heals for the damage dealt.
Death Incarnate. Any creature killed within 60ft. of the Angel of Death heals the Angel of Death equal to it's level or CR and cannot be revived by any means short of a Wish spell.
Spellcasting. The Angel of Death may cast the following spells without material component requirements (DC 20):
At will: Chill Touch (4d10), Sacred Flame (4d8), Toll the Dead (4d8/4d12), Inflict Wounds (4th level), Detect magic, Greater Restoration
3/day: Dispel Magic, Plane Shift,
Multiattack. The Angel of Death uses Soul Rip and makes four attacks with its Sacred Sickles, it can replace up to two attacks with Toll the Dead or Inflict Wounds.
Soul Rip. Charisma Saving throw: DC 25, Each creature of the Angel of Death's choice in a 20-foot Emanation originating from the Angel of Death. Failure: Target's take 9 (2d8) necrotic damage and The Angel of Death gains (9) 2d8 temporary hit points. Success: Half damage.
Sacred Sickles. Melee Attack Roll: +15, reach 5 ft. Hit: 19 (2d8 + 8) Slashing damage plus 9 (2d8) Radiant damage.
Necrotic Nova (Recharge 5-6). Constitution Saving Throw: DC 20, creatures within 30 ft. emanation originating from the Angel of Death. Failure: 26 (4d12) necrotic damage and cannot be healed until the end of their next turn. Success: Half damage.
Last Rites (1/day). Charisma Saving Throw: DC 20, one creature below half of its maximum hit points within 60 ft. of the Angel of Death. Failure: The creature dies. Success: The target loses half of its maximum hit points until it finishes a long rest.
Legendary Action Uses: 3. Immediately after another creature’s turn, The Angel of Death can expend a use to take one of the following actions. The Angel of Death regains all expended uses at the start of each of its turns.
Death Comes. The Angel of Death teleports up to 60 ft. to a point it can see, if it teleports to a point within 5 ft. of a creature below half of its maximum hit points, it may make up to one attack against that creature.
Death shroud. Saving Throw: DC 20, each creature in a 120-foot-long, 10-foot-wide Line. Failure: 18 (4d8) necrotic damage and blinded. Success: Half damage.
Penitent Souls. Saving Throw: DC 25, each creature in a 30 ft. emanation cantered on The Angel of Death. Failure: 18 (4d8) psychic damage and creatures' movement and damage is halved until the end of their next turn.
