Traits

Flesh of Unbirth
The Aberration cannot be killed if it has at least 1 active Shard of Uncreation within 300 feet. When it would otherwise be reduced to 0 HP, its essence will retreat to the nearest Shard, regenerating 100 HP and destroying that Shard. 

Fractured Creation
Each round, reality around the Aberration distorts. Within 30 ft. of it: matter is unstable; all creatures have disadvantage on Dexterity saving throws.
Healing spells heal only half as much.
Undead gain advantage on attack rolls and saves.
Actions

Genesis Claws
Melee Weapon Attack: +19 to hit, reach 20 ft., one target.
Damage: 6d12+12 slashing + 6d10 necrotic. If the target dies, it explodes in a shower of flesh (may activate Aborted Birth).

Primal Mouth
Melee Weapon Attack: +19 to hit, reach 15 ft., one target.
Damage: 8d10+12 piercing + 8d10 psychic. If the target fails a DC 24 Wisdom save, it is mentally fragmented for 1 minute (confusion, loss of identity).

Glare of the Unborn (Recharge 5-6 turns).
90-ft. cone. TS Constitution DC 24. On a failure, you take 18d8 necrotics and are Mutated (disadvantage on everything and halve speed) for 1 minute. Success: half damage. 

Multiattack The Aberration makes three attacks of its choice between Genesis Claws, Gaze of the Unborn, and Primal Mouth.

Unbirth Burst (1 turn). Dies by exploding in a wave of flesh; all creatures within 5 ft. must make a DC 12 Constitution saving throw or take 2d6 necrotic damage.

Traits:

Flesh Swarm Shards do not provoke attacks of opportunity. They can occupy the same square as other creatures.

Creator's Call If the Aberration of All Creation is within 120 ft., the Shards have advantage on attack rolls.

Radius 30 ft. – Area Effect

The aberration contracts matter and rewrites it into unstable, unwholesome forms.

Effect: All creatures of your choice within 30 feet must succeed on a DC 24 Dexterity save or take 4d10 force damage and have their speed reduced to 0 until the end of their next turn.

Fail by 5 or more: They take disadvantage on all attacks and saves until the end of their turn, as reality around them warps inconsistently.

Success: They take half damage and no other conditions.
Reactions

Overheat  Once per combat, the creature can overheat itself as another creature attacks with a weapon, to melt it. 

Overheat Duration: 6 seconds. 

Heat value: 20 trillion degrees Celsius. 

The heat doesn't affect anything external to creature, except for what is in contact with it. 

Legendary Actions

1. Claw of Wayward Flesh Attack Claws of Genesis.

2. Distortion of Creation Save Dexterity DC 24 or 8d10 necrotic and -2 to AC (1 min).

3. Call of Flesh (2 Actions). Causes nearby flesh to explode, generating 2 Shards of Uncreation.

4. Primal Hunger (3 Actions). Sucks in everything within 60 ft.: Save Strength DC 24 or drag 30 ft. and 10d10 force damage.

Mythic Actions

 

Revelation of the Broken Genesis.
All creatures within 150 ft. make a DC 25 Wisdom save:

Failure: 15d10 psychic, fall prone, and take permanent cosmic fear (immune to attempts to remove fear as long as the aberration lives).
Success: Half damage and no fear.
During the mythic phase: All spells cast within 150 ft. have a 50% chance to fail.

Description

There is no name, no concept, that the human mind can give birth to, capable of containing the horror of the Aberration of All Creation. It was not born, because there was no act of creation. It was not generated, because every womb would have rejected the obscenity of its existence. It is the primordial negation, the crack formed at the very moment in which God separated the light from the darkness and pronounced the first words of Genesis.

Before its thousand unborn eyes, the flesh implodes and the stone begs to be forgotten. Tentacles of aborted flesh writhe in blasphemous geometries, while bottomless jaws whisper prayers in reverse, every syllable an anomaly, every sound a blasphemy. Its presence does not devastate reality: it rewrites it.

Wherever it walks, Creation itself bends, corrupts, tears. The heavens darken, the stars fall to ruin, and matter—a poor, wretched thing—drips like flesh beneath a butcher’s blade. There is no will, faith, or steel that can stop what should never have been. It is the Primal Error, the Whisper behind the divine act, and it waits patiently for the day when everything will be returned to the dim abyss from which it thought it had escaped.

Lair and Lair Actions

- Rivers of Blood. All terrain within 120 ft. becomes difficult. Undead creatures are empowered (advantage on attack rolls and double damage).

- Awakening of the Unborn. Generates up to 4 Shards of Uncreation from the living earth.

- Reality Warping. Until the next round, all attack rolls against the Aberration have disadvantage and perceived distance is doubled.

- Storm of Failed Creation. Rain of Flesh and Bone: Each creature within 150 ft. takes 8d8 bludgeoning and a DC 24 Constitution save or Poisoned (1 min). 

If the creature dies, everything returns to its normal state after 1 minute.
