Old Bones. The Tyrannosaurus can't take the Dash action. After it uses Multiattack, roll a d6. On a 1-2, its Speed becomes 20 feet and it can't use Multiattack until the end of its next turn.
Half-Blind. The Tyrannosaurus has disadvantage on Wisdom (Perception) checks that rely on sight. A creature approaching from the side of its missing eye has advantage on Dexterity (Stealth) checks against it.
Starved. If presented with at least 10 pounds of fresh meat or equivalent food, the tyrannosaurus spends its next action eating unless it has taken damage since the end of its previous turn. A creature adjacent to the food can then make a DC 13 Wisdom (Animal Handling) check. On a success, the tyrannosaurus becomes non hostile unless threatened.
Multiattack. The tyrannosaurus makes one Bite attack and one Tail attack. It can't make both against the same target.
Bite. Melee Attack Roll: +10, reach 10 ft. Hit: 26 (3d12 + 7) Piercing damage. If the target is a medium or smaller creature, it has the Grappled condition (escape DC 16). While Grappled, the target has the Restrained condition and can’t be targeted by the tyrannosaurus’s Tail.
Tail. Melee Attack Roll: +10, reach 15 ft. Hit: 18 (2d10 + 7) Bludgeoning damage. If the target is a Huge or smaller creature, it has the Prone condition.
